What skills/standing do you need to achieve this?

Refining 5
Refining Efficiency 5
Metallurgy 5
Scrap Metal Processing 2 for zero waste reprocessing mods/alloys or the relevant ore skill to 2 for zero waste refining ores (ie Veldspar Processing 2 for zero waste refining veldspar)
Plus you need a standing of 6.67+ to reduce the station tax to zero.

Without Metallurgy and Scrap Metal Processing, your module refine will be 99.5%, good enough for me.

the formula is
station efficiency (.5 or .35 typically) + (base efficiency .375 * (1 + refining skill * .02) * (refinery efficiency skill level * .04) * (specific metal efficiency * .05)
so if you had refining 5, refining efficiency 4 and no specific skills with a 50% base station efficiency it would look like
0.50 + [0.375 * (1.1) * (1.16) * (1)] = .9785 or 97.85%
5/4/1 caps you
yes as mentioned you also need corp faction to remove the tax as well

add to this that little refining implant that reduce refining waste by 2 % and you got 100 %

When I first started playing EVE there were 2 people running around saying that they were getting over 100% yields. I always assumed they where just full of BS. Was there ever any inkling of truth to this?

Yes and no.
You can, on paper, get your yield over 100%, but you can't make something from nothing. 100% is the highest you can get.

Incorrect, Bex was correct with 6.67 (it's actually 6.666666666666666666666666666 rounded up).
5/4/2 or 5/5/1 skills is enough for ore for 100% refine @ 50% stations.
For scrapmetal you need 5/5/1 (because you need RE 5 to get scrapmetal processing 1)
