These are the main overall sources of ISK in the game. They are, infact, the only ones where it does not require another player to lose money. Please add more if you know of a source other than these three where players are never involved.
Again: insurance. For one, it happens no matter how you lose ships — whether it is to other players or not. For the other, the insurance payout does not require an ISK loss. You get (very) basic insurance without spending ISK, but I suspect that what you're talking about here is the loss of a ship, which is not an ISK loss…

No. Item loss does not in any way remove ISK. The losses you're talking about happened long before the item was removed: ISK was lost when the ship was constructed, not when it was lost; (very tiny amounts of) ISK was lost (through taxes) when the ship was sold, not when it was purchased and certainly not when it was lost. Buying an item does not remove ISK — it only transfers it.

Removing insurance does not add a "high-end drain" — it only removes an ISK faucet. Bounties is just a part of the isk generated. What really generates isk is insurance. Mining, doesnt add new isk to the game, old isk just changes hands. However when a ship dies, you get a nice sum of isk injected. 1 Insured and dead carrier is like 900 mil created.

I think the one thing we're ALL missing is that the EvE economy is so complex, that no single person has a full grasp on what it all encompasses. All that I can say for certain, is that it works. Is it perfect? No. But it's about as good as you can get without having a full company or economists instead of the one person they currently have.
Maybe in another 2-3 years (or longer) CCP will have slowly balanced out the ISK sink-drain to where they want it. It won't be perfectly balanced (see: closed loop economy where no new isk is added) but at that point it will be the best we'll ever get.

That is a highly undesirable thing.
You alwas want slightly more ISK to enter the economy than leaves it, assuming you have a fairly constant population growth. Otherwise you will end up with hyper-(de/in-)flation

The problem right now is that what create new ISKs can be farmed ad infinitum. We never run out of missions to do and belt rats keep spawning no matter how many we kill. CCP should consider tweaking that so nobody can do mission or ratting all day, or at least not in the same system all day long.
