I've locked down a wormhole at 100% but am unable to bookmark or warp to it. Did it collapse or something else?

Are you using four probes? Are they placed in a tetrahedron shape? Are you using the smallest possible scan range? Are you probes placed an equal distance from each other? Are your probes too close together? Is the signature at the exact center of your tetrahedron? Are you using a covops ship? Are you using sisters probe launcher/sisters core scanner probe? Are you using gravity capacitor rigs? Do you have high enough astrometrics skills?

I get that from time to time. It's a bug. "jiggle" the probes... seriously.. move them by a tiny tiny amount and then rescan.
if that doesn't work then do it again --
Nothing is worthless, you may have gotten it for free but it still has an inherent value

It's not really a bug. 100% can be less than 100%. Like a lot of things in EVE (such as sec levels) they are rounded. You may have got 99.95% which would show as 100% but you can't warp as the warp uses the actual value. The cartoon linked in the second post shows it a lot better:)
As said earlier with decent skills, ships and mods its much less likely to happen.

This is usually how I have to fix the problem also. Just move the probes closer to the point and you should be able to bookmark it after you scan again.

The interface keeps track of all 100% hits. If the wormhole goes away after you've scanned it down but before you warp to it, you won't be able to bookmark or warp to it because the wormhole will be gone. The UI should say something to the effect that the 'object has left the system'.
Relogging or switching systems will clear your UI's view of what is 100%. --

Can happen as stated, due to rounding.
The solution is to bring all the probes closer to each other and center the target in em. aka "Squish 'em" :)
I would however add that using a flat layout (racial invention data interface pattern) vs a tetrahedron will make things like that alot less common, and loads easier to manipulate...if that is what you are using.
