Within the EvE universe does life exist on the planets or moons? I understand the technical reasons why we don't go down to planets and interact with their surfaces (and possible dwelllers) but has it been worked into the lore at all?
Are there cities, towns, villages, farms.. microscopic life? Are the four playable races from the same ancestry (no matter how long ago) or are they actually from four different regions of space?
Is there a link someone can point me to regarding this, or is the information found in any of the books, or does it just not exist?

Some of the EVE Chronicle stories involve civilisations on the planets. Also one of the things in CCP's list of projects to complete is Planetary Interaction, so I'd say it is safe to say there is life on the planets/moons... and later down the line we should get to interact them a bit more.

Aaaand! If you look at some of the planets ingame, you'll see light from cities and roads and such. So some planets do.

according to backstory, Earth humans are only intelligent life form in EVE, and share ancestry. there are other life forms, but it's debatable whether these are adapted from Earth life, or indigenous, as 20,000 years tends to blur the differences.

You don't get around much, do you? Or perhaps your UI is so cluttered you don't actually look at the scenery? Anyways if you do try to fly around a bit (near planets) You'll notice that plenty of them show sign of human life. However, you might want to consider a few things, first of all; I now have one of those annoying sigs. second; you should probably move on to some more interesting things than reading this sig.

The four races of EVE are all descendants of colonists from Earth, something like ten thousand years ago, isolated and brought back to a medieval state by the collapse of the EVE gate, the wormhole that brought humans into this galaxy. They then each advanced to interstellar flight separately. All the bloodlines of EVE are thus simply humans, made slightly distinct by thousands of years of separation.

The creators of the sleepers, as well as the other "ancient" races, were humans brought through the EVE Gate in the same manner as the four empires. There is no known non-human intelligent life in the EVE Universe.

As others have pointed out, all humans in EVE are the descendants of colonists, that themselves came from Earth, and planets that had been colonized by people from Earth.
There are plenty of things indicating that there is life that doesn't have Earthly origins, but nothing with intelligence comparable to man has been mentioned so far.
Keep in mind, that many of the inhabited planets in the cluster were perfectly habitable when the first settlers arrived. It's likely that those early settlers would have killed off, or enslaved (Amarr victor!) any competing sentient beings.

According to backstory, the first novel, the in-game appearance of planets, etc, yes, some planets are obviously settled by humans, although the majority aren't habitable.
Yes, all the four races are descendants of humans from Earth. They came through a wormhole thousands of years ago, settled this galaxy, then the wormhole collapsed, stranding them here and civilization collapsed, and then later rebuilt itself into what you see today.
I don't mean to be rude, but maybe you should actually read some of the backstory? You're asking some very basic questions. You'll find it on the menu to the left under "EVE ONLINE" -> Backstory. Read the Scientific articles and some of the chronicles and you'll probably learn a lot about the setting.
