Somehow I think I became a member of an NPC Corp. called Viziam. I have no idea how this happened. I did apply to become a member of another corp (their name escapes me at the moment but they provide the ability to provide jump clones) about a week ago but never got around to leaving that one.
How can I leave this NPC Corp? Thanks!

When you first join eve you start in one of the rookie corps. When you joined a player corp you automaticall left this starter corp. You were more than likelly kicked from the corp and now you have ended up in the default corp for your bloodline in your case Viziam.
To leave Viziam you just need to join another player run corp, but if you should leave this you will end up in Viziam again.

That would be my corp, EACS. The jumpclone service I offer has only one limitation, and that is that you have to leave the corp within 4 days. About 80% of the users leaves on their own to other player corps or a NPC corp, I kick the other 20% manually. I used to send people who were kicked a message explaining why, but that became too much work as the number of users of my service grew. It should be clear anyhow, as the 4 day limit is clearly advertised.
People who leave a player corp (on their own or being kicked) without joining another player corp are placed in a NPC corp based on their race and bloodline.

How is the tax rate for this, or any other corporation, computed? It's at 11% for this particular one. Is that 11% of my balance that's taken out? If so, how often?

11% of your rat bounty and mission reward/bonus income.

Think of the NPC corp as a place holder, it does nothing but hold you until you join the militia or join/create a player corp. ____________________________________________ When going to a party with wine, women, and song. Always ascertain the vintage of the first two. |

Estel would have kicked you as you would have affected his corp standings after several days which would have messed with the clones he can provide and been a potential disaster. His corp is to provide the clone only which would normally take a lot of effort for a player to get.
Player corps can set any tax they want. 10% seems normal but not set in stone. NPC' corps are fixed at 11%.
In EVE you must ALWAYS be in a corp so the game provides one for you (an NPC corp based on race and a random factor) if you're not in a player corp.
What is taxed was answered.

11% of your rat bounty and mission reward/bonus income.
This, with the additional limitation that only payouts of 100k ISK or more are taxed. If the mission reward or bonus or the batch of rat bounties (rat bounties are paid out in batches every 15 minutes or so) is less than 100k, it will not be taxed.
Corp taxes do not apply to market transactions, so there is no corp tax on income from mining, looting, salvaging and trading.
