(1) Most of the factions openly supporting slavery come from Amarr. The Khanid Kingdom and the Ammatar Mandate are essentially Amarr, as are the Blood Raiders (Amarr heretics). The Angels and very likely other criminals do however do human trade where ever it is profitable, so yes, there are factories ran by slave labour in the Federation. (I think there's a short story about a drug plantation in the Fed.) And finally, the True Slaves of Sansha's Nation are mind-controlled slaves.
(2) Most legal Amarr slaves are these days children of other slaves, with family lines running back to a huge multitude of people conquered by the Empire. In addition, captured criminals and prisoners of war may be sentenced to slavery. At times, "criminals" may have quite a loose meaning, and even minor Holders (nobility) can be sentenced to slavery (there's a chronicle, Fait accompli, featuring an enslaved Holder).
In the Empire, major slave races are Minmatar, Ealurians and Ni-Kunni, though there are multiple unnamed slave races, in addition to enslaved individuals of the major "free" bloodlines, including True Amarr, Udorians and likely even some Khanid, though as a whole, True Amarr (the original ones) and Khanid have never been enslaved.
(3) As expected for an Empire spanning 40 % of civilized systems, there's a multitude of sects in the Amarr Empire. Considering slavery, the major factions are some sects and then the major Royal Houses.
Royal Houses: - Ardishapurites (followers of the Ardishapur Royal House and very strict Orthodox faith) would be expected to quite strict and "backwards". They now control the Ammatar Mandate, too. The Heir may be an Orthodox believer (and possibly only real believer amongst the Imperial Heirs), but she's very intelligent. - The Kor-Azorites (a Royal House known of their diplomacy) has been liberal due to their previous Heir and the late Emperor Doriam II. - The current Kador heir seems to be quite liberal, but his house is very traditionalist. - Khanid would be expected to be a mix of traditionalists willing to keep the status quo, though they are very eager to use new technology not yet accepted in the Empire. - The Sarum are the militarist royal family, slave handling likely depends on the function of the site. I'd expect very strict control on slaves. - The Tash-Murkon are the economical powerhouse of the Empire. They are also (as the Udorian, not True Amarr royal house) the champions of the liberal cause. Stereotypically the slave handling practices would depend on what is more profitable.
Sects etc: - Tetrimon: Apostolicals, they wish to return to the old ways. This is confrontational to the Royal Heirs and the Theology Council. They do advocate very strict handling of slaves. Currently the Cult of Tetrimon is banned in the Empire. They may be allowed to operate in the Khanid Kingdom. - Stellar Staircase. Appears in some missions. Compare to the Underground Railroad in America. They are Amarr actively working to undermine slavery and illegally help escaped slaves. - Blood Raiders: Depending on the exact sect or cell, they are not very strict on pure-bloodedness (Omir Sarikusa, the leader of the Covenant, is not True Amarr IIRC). However, they do engage in illegal slave trade, and likely raid the Republic as well.
