Oh there is so much misinformation in this thread. I have done hundreds of these. It works like this:
Cosmic Anomalies: Can be found with Onboard Scanner or Probes, and depend on the Security status of the system (i.e. no Sanctums in hisec). The OP posted a list of these. These have a *very* small chance of escalating to a DED complex, with no in between steps. This will create an entry in your Journal that tells you the system which it is located in. It is reserved just for you like other escalations, and cannot be scanned down by other people (though you, in your shiny lolRaven can be). It is less than 5% chance, and in my experience is more like 1%. Havens and Sanctums (which I run primarily) escalate to 10/10 The Maze (verified, personally). I do not know which Anomaly escalates to the 6/10 (Gurista Troop Reinvigoration Camp), as I don't run the lower ones much at all. I have been told it is the Hub, with no way to verify this.
Cosmic Signatures (combat sites, specifically): These require scan probes to find, and will show up as "Unknown" until your signal strength is high enough to display their name (wormholes also show up as "Unknown"). There are two main types of these, as they were added at different times in Eve's evolution and tweaked over the years.
First are the DED rated complexes, such as the 6/10 Gurista Troop Reinvigoration Camp, the 8/10 Pith's Penal Complex and the 10/10 The Maze. These are all one stage (multiple rooms)long, and have an objective at the end, either an Overseer npc or a structure that drops loot (sometimes) and Overseer's Personal Effects (19th Tier, 21st Tier, and 23rd Tier respectively) every time. These never escalate.
The other type are not DED rated. Examples are Gurista Base, Gurista Fortress, Dread Guristas Fleet Staging Point, Guristas Military Complex, etc. These have a chance to spawn Dread Guristas ships at various points and a chance (which varies depending on the site) to escalate into an Expedition chain. Each new point on the chain is several jumps away (random) and has a chance to escalate further or not at all. If you make it to the end of the chain, you will have a chance at some deadspace loot, or (often) nothing at all other than a Dread Gurista Tag and some faction ammo.
I used Guristas examples, but it works the same for all factions.
