Many US banks have recently started charging 'foreign transaction fees' that serve no real legitimate purpose on any transaction that even LOOKS like it's going out of the US. The fact that paypal charges it to your account as 'CCP Games UK' is enough to trigger the charge, even though the bank itself is actually paying a US company(Paypal).
It's a blatant money grab, one more attempt on their part to steal their customer's hard-earned money for doing literally nothing useful to society, because their profits are down now that many of their previous fees like that have been outlawed or severely restricted.
You can PROBABLY get around this by doing it indirectly: Having paypal withdraw the money directly from your bank account and then passing it on to CCP. Slight pain in the butt, but in theory the two-step process should prevent your bank from seeing that it's ultimately going to a UK/Icelandic Company and thus ditch the charge. In theory.
Ultimately, since it's a transaction in US Dollars(no currency conversion involved) and is going to an English-speaking company, they literally don't have a legitimate reason for the charge. Using Paypal USED to bypass it, but doesn't anymore for most major banks.

Also I would honestly trust CCP more then the US banking system since the banks will find anyway possible to rip you off cause they know they can get away with it.

sounds like the $19.95 charge was for a toon that was a trial that you turned to a full version toon. If not it is possible that your bank is charging you international fees. Some banks/cc companies have a base international fee then a conversion fee

Confirming that the OP's bank is far less trustworthy than he has been led to believe. My financial institution at least has the balls to label it clearly as a foreign transaction fee. ----------

Most banks and credit card companies charge a few cents more for transactions. They are normally hidden in the cost of the product you buy, or shipping charges.
If it is a transaction from another country some companies add a small percentage on to the bill to take into account the fluctuation of the money markets.

EDIT: Actually, let me make a correction. The bank isn't the one who is levying a foreign transaction fee. Both Visa and Mastercard charge a 1% fee for any foreign transactions, regardless of the bank which issued you the credit card. So the untrustworthy institutions are Visa and Mastercard in this instance.
According to this website (updated last in November 2008), Capital One is the ONLY bank which does not charge a foreign transaction fee at all. Visa and Mastercard still charge a 1% FTF, but Capital One absorbs the fee instead of passing the charge on to their customers. Of course, this means that the customers probably have to pay for this fee through other means, such as higher interest rates, annual fees, other service charges, etc...
The trick being, that most banks don't simply charge Visa/MC's 1% fee, they add a few % on to it, I've heard in some cases it can go as high as 5%. My bank, a few months back, charged 3% on an eve renewal fee.
So yeah, Visa and MC are doing some greedy money grubbing, but the majority of the charge is generally going to the bank, not to the credit card company.
