I know that increasing refine and refinery effeciency and also the particular ore proccessing skills will reduce wastage. But can someone give me how this formula work?
And where to look for the info to do the calculation in the game?
Basically I want to maximise my yield in reprocess.

i dont know exact formula anymore but, some helpful tips to remember
station equipment at lower than 50% you will need to get relative ores higher than level 1
refining and ref eff will reduce waste factor
we take is reduced to 0 when your standing with corp your are refining with reaches 6.67 or higher
besides the relative ores, in order to get 100% on loot (modules), you will need metalurgy and scrap metal processing skill(i.e. to get the pesky 1 zyd from x-ray crystals)

So it is possible to reduce to waste to 100%? I mean we are able to get 100% what the ore is suppose to reprocess to?

100% Refining/reprocessing at a 50% station is very possible.
It requires a few things.
1. Standings of 6.67 or higher (removes the 5% they take)
2. Refining 5
3. Refining Effeciency(sp) 5
4. Specific reprocessing skill 2 (ie Veld reprocessing)
this is the old refining formula.
Yield = .75 x (1 + .02 x Ref skill Level) x (1 + .04 x Ref Eff Skill Level)

Your-Skills-Percentage = (station_equipment + (37.5 * (1+(refining_skill*0.02)) * (1+(refinery_efficiency_skill*0.04)) * (1+(processing_skill*0.05))))/100
Note:Your-Skills-Percentage has a maximum value of 100%.
You Receive = trunc( Max-Mineral-Yield * Your-Skills-Percentage * (1 - Station-Take-Percentage) )
We Take = trunc( Max-Mineral-Yield * Your-Skills-Percentage * Station-Take-Percentage )
Unrecoverable = Max-Mineral-Yield - You Receive - We Take

actually standing of 6.67
and the following skills
refining lvl 5
refining eff lvl 3
specific ore processing at lvl 2 'll give you 100% yield,
unfortunately not all ore processing skills are available at lvl 3 ref eff so might be better to get ref eff lvl 4 and ore proc skills to lvl 1, should give the same effect

all above is correct, tho most refiners forget that scrap metal processing is the skill needed for module and ships at 100% as well as the above skills  and ofc ice processing for the much more needed pos fuels

This is not correct. You will receive all the minerals from a refine/recycle with skills and station equipment that yields 100% and a standing of 6.67 or above with the corporation owning the station.

I have refining 5, refinery efficiency 5 and scrap metal 1 and I get 100% from loot (Standing of 9.9+ where I refine).

well my char got refining and refining eff 5, scrapmetal 3 and still i'm loosing 1 to every stack i reprocess. don't have the 6.6 standing.
but what's odd is that untill last path i didn't had this, but heard people complain about it as well.

Just to confirm, lets say if I got refining 5, refining efficiency : 5 and station standing above 6.67.
If I refine Plagioclase, will I get 100% or I need to learn the particular ore processing skills as well?

You will need to learn the particular ore processing skills as well. You will get 99.5% without them.
Edit - This is only for stations with 50% equipment. You can not get 100% at a 30% station.

perfect ore refine at stations requires Refining 5, RE 3 and Ore skill at 2 :p
For perfect component refine you need R 5 RE 5 metalurgy 5 and Scrapmetal 1

You will need to learn the particular ore processing skills as well. You will get 99.5% without them.
Edit - This is only for stations with 50% equipment. You can not get 100% at a 30% station.

Yes, that is at a 50% station.
Refining 5 and fefinery efficiency 5 give 79.5% at a 30% station. The maximum yield (the specialized processing skills at level 5) at a 30% is 91.875%.

at refining5, reg efficency 3, and the processing at 2, you will get only 1 unrecoverable.
Not really worth the extra week few days of training to get that last 1, so if your in 50% efficency stations, stop there.
If your working outta 30% stations, then it's in your intrst to go further
