The funny thing is that many of these human errors cam be anticipated in advance through correct interface design of the user and the engineer. Keeping data on the right screen, distinct colours and interrupts in the right places can work with human behaviour patterns to anticipate and work around the issues. Human Factor Methods, its being driven by that litigious society.
It's something beautiful about humanity in a way, make an other wise individuals issue with the world into one of the group, give it impetus and a bit of time and we find entirely new ways to work for the benefit of that group. The downside that we see every day is people being annoyed and inconvenienced, but under the hood people are living longer, better lives.
Point in context, when coming to a crossing there is a slight slope towards the road and concrete dimples. If you were blind, this is a tactile cue about the environment, mildly annoying for the minority, but essential to a somewhat disabled minority. Take on things that do not effect us, make it our collective problem and we advance as a society.
With regards to the "may contain mushrooms", consider this. If you are not the person who will be eating it, a trick of human memory reclectin is to make a statement, with which memories are associated. Say "allergy mushrooms" there's a large chance you will remember that being associated with someone you know. I have to deal with this with gluten (barley, wheat, ect) because a friend developed it. Took a while though, it didn't occur to me because I was looking specifically for gluten or wheat, not the other sources so for these reasons amongst the legal get out clause is why it is included.

Honestly the mouth breathers that dont understand that a bottle of milk has milk in it should definatly be Darwined by their own hand. But sometimes we have weird situations about the opposite, too. I think I saw a cheese here where it said: "Contains: cheese (97%)". And I was "Wait..so what about the remaining 3 %? It says nothing else- what about the 3%? What the hell am I eating here???"  Depending on the quality, it would be either rind from the stomach of a goat or sheep, or wax to seal the cheese and keep it fresh.

Why do they have ingredients list on bottled water? It clearly says the ingredients on the front of the label.
Oh 'merica, you so crazy. Legal requirement all human consumables be labled in their entirety with all references to key allergens and sensitivities. Make an exception for one, you risk everything being labled Water with a load of something else.
