I would like to thank the GM's who handled both my petitions over the last few weeks.
It has given rise to one of those catch-22 situations(Mentally that is) On one hand I am pleased to see the ships I lost refunded, yet on the other hand I cant help but wonder when it will happen to me again.
Perhaps by posting here I ill get an idea of how common the issue I had is and perhaps find out how to avoid it in future.
First: The History.
I have no LVL 4 agents of my own unlocked yet but I often accompany one of my corp-mates on his. We had done several of the easier ones before being presented with Silence the informant then Worlds Collide. Now I have not got the strongest combat character in existance and I am not yet able to solo hard missions of that level, my corp-mate is a stronger combat pilot by far and he can manage most of em with a few warp-outs. Together however we are able to lay waste to most of the missions we are presented with. I can make the old tank last a bit by using booster 800's and my mates Raven has shields streching round the block and back. However we do have the policy of trying to escape before the tank breaks, which most will agree is a good idea.
So in the case in point we alligned for warpout and set full speed ahead. The rats pounded us in their usual fashion until our main tanker had to go. Fine ....no worries so far, I pick up the aggro from the many many nasties and activate my hardners.
After a minute or so of concentrated aggro the *****s start to show, the dps is only just outdamaging my reppers ability to cope, so in plenty of time I initate warp-out. Then it happens: The UI shows my ship go into warp...but. Well you know that moment when the warpdrive kicks in? When everything stops for a half second before you warp off? Well I seemed to get stuck in that moment, along with another corp-mate in his Prophecy we we sat there watching the damage mount and mount, it is a measure of the effectiveness of my setup that I was able to hang in there for almost a full minute, all the while being pounded by the rats till my Megathron went pop!
I entered a petition and in a week I had my ship refunded(After reading the ship loss FAQ I doubted very much that I would see it again) I made the mistake of buying myself a Domi, so when the insurance money was taken back I found myself low on cash. Great. I have my ship back everthing is peachy.
Then last week the exactly same thing happened again. I have just recieved confirmation that my second ship is also being refunded so it is definately some form of issue. Once again I wish to thank the GM's for their response, just looking at the number you are assigned as reference gives you an idea of how busy they are.
BUT..... What is causing it to happen? Has anybody else had it happen to them? Even though we have run other missions together since the incident I cant help but wonder when(not if)it will happen again. On a side note, the guy whose BC was destroyed was already on the point of leaving EVE and we have not seen him online since the incident, so he probably doesent know his ship is back... poor sod, we invited him along as support to get him back into the game again and look what happened.
The point of this whole giant diatribe is to ask you all if this is avoidable, and if so, how to avoid it. Does it only happen to Megathrons?

This is an issue that a few people have had. It's not ship dependent (I know of a Raven, a Tempest, and I believe a Carrier that have had this problem). I don't think anyone knows what causes it, but the Dev's are aware of the problem and should be working on a fix.

I lost an Apoc to the same thing. Aligned, going forward, engaged AP with 40% armor left and then..... nothing. I was not scrambled as warp had started, I was slowing down and moving out of alignment even though the stargate was infront of me.
There is something worrying about seeing that last bit of cap go and then structure followed by "Your ship is out of control".
I petitioned and also bought a new Apoc, 2 weeks later I was 40m isk down but had my apoc and all lost fittings back. Good thing was that my corpmates picked up my can so ended up only 5 drones short and a bit for some repairs of the damaged equipment.
Screenshots help alot in this case as it showed what was going on and makes it easier to track date, time and location (all marked on the screenshot). << Insert whitty sig here >>

I have lost a wolf to an incident like this, and I know of many others who have had the same thing happen.
There is also the issue of your client showing your warp out, and you still have the npc's pounding on you from 14AU away untill your ship goes pop at a planet or wherever you happend to warp to.
It will happen again. Expect it. Hopefully it will get fixed and hopefully while we are all waiting, if we lose another ship due to this, you will still get your ship refunded and not be an unlocky sod like a friend that lost a BS and couldn't get it refunded.

In a way its both reassuring to know I am not alone....and worrying that I am not alone.

I just lost a BC to this bug I think :( Doing agent missions also.
I was low on cap but not shields so it was time to warp out, but I got stuck at 100% on the warp progress bar for at least 30 secs or so. Fully aligned, no obsticles.
Not scrambled, I dont think those NPCs can anyway.

hi people, i have had the same issue and have recently lost 3 or 4 brutixes to warp out lag......so i started using a ping program that allows me to gauge what is going on with current lag and find that it is best to relog and restart my internet connection before sensitive missions if i start "hiccupping".
note that my connection is probably the worst in the world,(besides south pole station) and such drastic measures may not be needed by most. i mostly run windowed so that i can hit the start button whenever i feel that something goofy is going on....in almost every case, relogging clears it right up and i have no problems with finishing my objective.

yes it's a known issue and the Dev's *cheers guys* are responding swiftly and nice:)
It looks like this bug occurs especially in the "silence the informant" lvl 4 mission
(i also lost a machariel in that one alligned and warping from 40%+ shield till boom) *started warping early as i saw i can't compensate the damage and still got wasted:P*
i found that the 2nd stage of this mission is rather easier solo than in a gang, since it looks like the 2nd man in the gang gets FULL aggro as soon as he shoots anything on the stage :)
3 wingmen and 5 commanders (initial aggro) are not THAT hard to manage...the whole stage is another story :P

A note for others. They cant reimburse the can that drops as loot, so if its worth picking it up then do so. Also, screenshots would have helped as they can show that you warped out in plenty of time.
The only thing is that I couldnt wait and I purchased a new cyc to replace it already. So I now have a spare insured cyclone  
As for the bug re-occuring, I am simply avoiding the situation by being carful and rejecting the harder level 3s until I am sure I can cope without warping out. I dont PVP much but i suspect that it isnt as much of an issue as the looser usually gets scrambled and cant get away anyway (most of the time)
