I changed my appearance months ago, but my avatar is the same here. 
What do I need to do to get it to update?

Step 1) Buy ticket to Iceland.
Step 2) Travel to CCP HQ.
Step 3a) ***** yourself out to Oveur as his (sex)slave.
Step X1) Stop at liquor store between your house and CCP HQ.
Step X2) Buy vast quantities of alcohol (it's probably cheaper to do this somewhere else than Iceland).
Step X2a) Rent large cargo plane/truck (see X2).
Step 3b) Throw impromptu party for devs.
Step 3c) Ignore 3a.
Step 4) Enjoy new portrait.
Good luck and be wary of elves.

Almost 3 months now, 2.7mil SP, no avatar yet.
Avatar creation probably requires forum w*****g?

Probably. I recall the devs stating something about how every downtime a few more portraits would get processed until the backlog was done. Of course in true CCP style it appears that this isn't happening.
Frankly I'd rather they just completely fix the drone scoop bugs, it's better than it used to be but still bugged. The extremely simple and obvious fix of increasing scoop range has not been undertaken, sigh.

Go to your eve folder.
Go to cache, and clean it all out, becides the ''setttings'' folder and the document (or whatever it is) called ''prefs''.
That makes you reload pretty much all avatars.
If it still isn't changed, wait. It took CCP about 2 months to change my picture on the forums, even while the in-game one was instantly, so expect to wait even longer on that.

It's best to just ignore the whole forum avatar thing, its been screwed up for a while and I'm not sure the devs have ever said why.. Though maybe they have and I just don't know about it, which is also entirely possible.

the game and the forums are not the same thing, the pictures you see ingame are not automatically uploaded to the forums, they must be processed and are only done during downtimes when nothing of higher importance is being done.
and with the large influx of new players it will take much longer for new people to get thier forum pics.
(edit) sorry meant to add, that was the devs reply to the whole avatar thing.
