According to my Refining Level 4 skill, I should get an 8% extra yield on my refining.
The factory's base yield is 50%, but with my skills, I'm at 90.5%.
Training to Refining Level 5 upped the yield to 91.25%.
And now with Refinery Efficiency Level 2, which ups yield by 4% per level, I'm now sitting at a total of... 94.55%
Long story short, I am extra confy00zed. 
Just how exactly is the mathematics for refining calculated? Assume no tax (to simplify the formulas...)

From http://oldforums.eveonline.com/?a=topic&threadID=305662 we get the following equation
[Station Equipment] + 0.375x(1+[Refining Skill]x0.02)x(1+[Refinery Efficiency Skill]x0.04)x(1+[Ore Processing Skill]x0.05)
Note that the equation tops out at 100%, to avoid recycling and creating items for profit 
So at a 50% station, with no skills you get 87.5%
Given R4, you have an 8% increase of that 37.5% that is from you, so 40.5% of the total, giving you the 90.5% you saw
Given RE2, you have a 10% increase from R5 and a 8% from RE2, giving you a 44.55% increase over 50%, landing at the 94.55% you saw
Expanding this out, we get a total of 49.5% from RE5, meaning you need at least one level of the ore specific skill, plus RE5, to get a perfect refine (100%)

Don't forget that your standing with the corp that owns the station affects the return on refining. I have R 5, RE 3 and the common ore skills at 3 or 4 for the mining crystals and I get perfect refine.
I think the required standing is in the mid 6 range.

About 6.7 standing to the station owner should get you no skim off.

Unless CCP has changed it since WAY BACK, when I trained the refining skills, you actually need Level 1 in Scrapmetal Processing.
I had a lot of the ore skills at a high level, but still only got 99.5%. Soon as I trained for Scrapmetal, I got to 100%...
But again, that was way back, more than 2 years ago

RE5 is NOT needed for a perfect refine. lvl3 in an ore specific skill and RE2 would be enough. Though it would not be a mistake training RE5. It is needed for scrapmetal processing anyway. :P
