Although I am unsure what circumstances enable its usage, you may be right with the corrupted cache idea. As for the goonfleet image, theres currently no way to check for modified images such as those even though they have been used, at least by goonfleet, for a long time (check this and this) You take what is offered. And that must sometimes be enough.

I believe that (blue) picture symbolises a Goonfleet member. There are no placeholder pictures like Wildwa in the OP. The place holders are the pictures with a stylized person. If you DELETE a picture in your cache folder while in flight, and then the client tries to show you the picture again, the picture will be a very weird combination of colours in stretched lines and stuff. Had a bug back in Castor when the client couldn't put lighting to generated portraits, so I tried deleting pictures while in flight to reload them - it didn't work. - Three years old

It works simple: There exists a database of players online. (characterID, corp, characterName) This information can simply be gained, from visiting any trusted site in the Eve IGB Hence, you can construct a database of friendly pilots, if everyone in your corp/alliance visits the site. Now, portraits of pilots are stored locally in your Eve install folder, surprisingly in a folder called "portraits". These are all PNG files. Can you see where I am headed yet? All that needs to be done, is to install a 3rd party app, which looks at this online database, and pushes custom pilot portraits into the portrait foler. The first few steps (using IGB) can even be avoided if you understand how the machoNET cache works. This is not only an exploit, this also allows people to gain a significant tactical advantage. How many times have you been ganked ot lost an easy kill, simply because you cnanot track the 400+ names of friendly pilots? Anyway, although it's an exploit, it's also frightningly easy to do (look at theintelproject.net for another example of harvesting characters). What I do the rest of the time - Vote for a Jita bypass!

I heard goonfleet uses program sitting between client and eve server and modyfing character pictures sent to you so you can see standings without getting info. I am not sure if it's exploit or not. They are not modify client but they are changing communication on the fly. All this info you can already get from inside the game just by clicking on a person. I think it will be better if CCP would inform people about known exploits (just like they did with Wasp drones). I think all channels should give you info about standings right away. It's the key to eve politics and currently standings code is really bugged and underdone.

There's a folder with portraits there where all your portraits are saved so you don't have to load them everytime you play. If those get edited then you'll see the edited portrait in-game. That's what I think has been done on the first pic. Not sure about the second, since it seems to have ALL the pictures saved in it's cache so that might be some sort of hack...
