I strongly feel its time to remove the insurance umbrella for certain actions.
Basic payout should be the maximum amount given, even if you have the "platinum" packet when/if:
* You lose your ship while engaging in criminal acts aka Conkkordokken. * You lose your ship in 0.0
Personaly i would love to see a non insurance policy for these two circumstances, but that might be to much. But as a first step a basic payout will be ok i think.

A bit off topic. Does a Mothership and Titan have a basic insurance? Since you can't dock = no insurance but do you get *something* like when non-insured ships go *pop*. ______

umm no maybe it would make people think twice before running around without a scout and such.. its lawless space so why should insurance cover losses there, same thing with concordokken

Yeah, kill 0.0 as we know it, but a new will arise.
On the "change insurance" wagon, I think the insurance time should be reduced to a 6 weeks tops. 3 weeks would be even better.

bit of topic but who nova days insure they t2 ships anyway? Tbh insurance in eve is one big joke of many others, i think ccp must to look for other solution for insurance system cause the way how it is now is simply silly.

I believe insurance should be done away with entirely.
In rl insurers specifically exclude losses arising from criminal acts and from acts of war (among other things).
Just why would there be insurance at the rate of loss pod pilots get their ships blown up I don't know.
Aside from these logical concerns, from a gameplay point of view I agree with the OP that ships destroyed by Concord should not get insurance payouts. As for ships in 0.0, I don't feel as strongly about it, but I would tend to agree on that one too.

Most people agree on this. I'm not sure why CCP doesn't implement it. Maybe they will, or at least give some comment as to why they don't. Maybe they can't figure out how to code it.

For this, I believe a cap would work better. (For example, no insurance payouts exceeding 10M isk.) That way there's no complaining about new players getting concordokken by accident and losing all their stuff.

I'd rather see insurance mails send out every downtime to be honest, instead of opening the insurance panel, and discovering 5 other insurances have expired...
