For me, it would be forgetting to put my AB on so i don't close the distance for scram range and my prey gets away (happend a few days ago) so what about any of you? biggest mistake you made that either resulted in your prey going or you dying?
(another would be forgetting to turn on hardners in a 1V1 BS fight (only won becuase of skills :D) and also fogetting to tell my heavies to atack XD)

Forgot to turn my mwd off and wondered where all my cap had gone when i tried to rep would be one iv done a few times :)
forgotton to put drones in the drone bay AND forget to load cap boosters into cargo.. i died pretty quick lol

Ah i rememberd another one, I followed a caracal into a system and opend fire, only to realise a split second later it was a high sec system :D my thorax lasted 2 seconds XD.

Thinking that I was in 0.4 space when in reality I was in 0.5. (0.4 was the next system)
Found a hostile and engaged. Things were going well, he was going down pretty fast when all of a sudden:
- Own shields gone.
- 90% of armor gone.
- I go pop
- I notice all the CONCORD ships surrounding me.
- Notice the "Ha ha ha" comment in local.

On an old char of mine, I was killing "veldspar" and was having a few drinks at the time. Woke up the next day with a bad hangover, beer all over my desk and my char was minus 1 covetor. Was a long time ago and still cant remember actually loosing the ship but I did. Not a piraty post but still sucked.

Warp scrambling the billboard, attacking in .5, mistaking a deimos for a thorax, I've done every stupid thing you can do.

almost first ever PvP fight.
I think, this is a rite of passage for noobs as it goes. My mate did it when he was a few days old, albeit in much cheaper ships, and without knowing the agressor rule .
Was in a balckbird, trying out the recon probe launcher in TAMA, interesting the things you can find in TAMA.
Anyways, found nothing that time! and thought, hell lets one jump around the belts and head home. First belt I see I warp in at 50K. There sits a caracal. Ive got a couple of ECM jammers on I havent tried out, I'm miles away from web/scram range. I thought I'd engage and see how it went, being able to warp away if it goes awfully wrong.
Well, it wasn't too bad. But, I jsut fired at him. All thumpy heart and hypertense. Adrenaline on it's way.. forgot my ECM's untill after his first volley hit. Whacked 'em on, but to be hoinest too little too late. Next volley hurt a lot, and my missiles wern't doing much.
So, as planned, I warped out to safespot. Calmed down a bit, waiting for the fingers to work properly again.
After a couple of minutes I warp to the gate.
Pop!
DAmn! I grinned though, I shoulda know better. Oh well, off to station to get Caracal out and see if can find chappy still in the belt.
goto station. Have 1 unfited caracal. Ended up fitting with most t1 and a few named bits. prolly 2-3 times more ISK than I would normally but heyt, time is pressing . dude might go home and not let me kill him.
Sorted! UNdocked from station.
Noooooo! I was clawing at my own face in horror as I missed the 'abort' and ended up undocking while still flagged.
Pop!
quickest I ever lost a blackie and acaracal together ;P

Mine was jumping on a Covetor that was mining in a belt in my lovely blaster harpy, only to discovery it was a .5 system. Needless to say it went boom rather quickly

I was harassing a pirate in a BC (drake i think, but dont quote me on that, it was a while back) in my hound
I was irritatating enough that he was chasing me from belt to belt (had him down to half armor even with his rep going he was still going down) when I accidentally warped to 0 on the next one instead of a random distance (he had 4 buddies in system hunting me)
they were just warping out but I knew I had to move out FAST or they would get me, so I tried to warp again and did the same thing, right into the middle of where they warped to
needless to say the af and inty had me locked down pretty fast, but i lasted long enough for the 2 cruisers and the BC to warp in and get some shots on me 
that was my first hound too... but it was just junk fitted from stuff I had lying around so not too big a loss
I'm currently involved in medical research, concerning the therapuetic aspects of a swift kick in the rear

Not sure if this counts, but here's the story, of what I was able to piece together over the next few days...
I'm sitting there, and I have a day off finally, so I grab myself an ambien and down it with some water. Let me give you some advice, if you do this and decide to keep playing eve... um, have someone take your power cord from you first and hide it...
Well Eve began to get really 3d... I mean really 3d, it was like I was inside of the actual universe, a totally trippy awesome event. Anyways, I'm in a Rifter, and completely unable to function on a logical level. Flying around Amamake, I somehow stumble into belt 3-1. There were 2 ships going at it, must have been a personal thing because they both completely ignored me, that, or I was in a Rifter and they were in T2 fitted BS's.
Well they are both right up on each other, and I start talking in local how it looks like they are hugging each other, and how I wanted a hug. No seriously, I was like almost crying I wanted a hug so bad :( Well I lock both ships and decide that I'm gonna help the guy who is winning. I start unloading all 3 of my T1 autocannons at the guy who was obviously going down. Well, a few of his drones focussed on me for some reason, and I go pop. I swear I didn't even see his drones, but they were on the KM the next day.
Now I'm in a pod, in low sec, in a roid belt, watching these two BS's go at it, and I enjoyed the fireworks as the one I shot at exploded. I'm now flying around looking at the loot from all the wrecks around, and complimented the winner on his victory, and asked if I could have some of the loot. I'm pretty sure I wasn't being serious, and if you can slur typing, I think I was doing that too.
Well I got denied, and asked for a hug, and got denied again. At this point, the guy tells me to leave or he'll blow me up. I swear I started to cry, because I sh*t you not, I was friggin lost. I told him how I was lost and couldn't find my way out of the asteroid belt. I kept bouncing off roids in my pod trying to get away. Eventually I docked up in a random station and went to sleep.
Talking to the guy the next day helped me clear a lot of this up, and he was pretty helpful and had a good laugh at my state of conciousness.
All I have to say is thank god it happened in Eve, cause if it happened in the street I would have gotten stabbed.
Don't do drugs and stay in school.

well... I have had so many.. engaging without extra ammo.. forgetting cap boosters. Forgetting drones.
My fav though is when I jumped headed to lowsec for some fun, and saw a covetor in local. Warp in, scram, web, attack... Pop him. Start on his destroyer alt.. get him to hull and WTF.. concord. Stupid 0.5 space.
Even better, the dessy alt scooped my loot and gave it back :)

My favorite was misunderstanding comms and thinking we were supposed to jump to the next system over and attack. I warp to the gate and find myself in the middle of the incoming gang 1 Manticore down.
I wake up in my pod 20 jumps away, clone jump to the station nearest the battle, and rejoin the fight in my Malediction. Only to realize it had no weapons. I managed to scram the primary though. Later, leaving the area, I got popped and podded. I never updated my clone 8 days down.

my first 1v1 with a command ship, that I was going to win. he used ecm drones which jammed me, but before the jam, i had my drones attack his (astarte versus pilgrim). my drones killed his, amazingly. Re-aquired target lock, ran my nos / neut / armor rep and TD's.. got him into hull, and he warped away
forgot to warp disrupt him, heh

My biggest one so far was like this:
I got bored so decided to do some hunting, went threw a neighboring .4 system threw the belts, there were quite a few belts did my scans and etc.. this failed so I chilled at the gate... so I got bored of this and gave up. Then out of no where this ishtar/drake combo attacks me.. I attack the drake hell who gonna take an ishtar in their drake? :P. So we duke it out for bit they give up and run off...
Fast forward few mins later an deimos shows up... am like am not gonna engage a tech 2 ship with sentries on me so I wait... he attacks me... with all the excitement of another tech 2 ship under my belt taught I engage and forget to scram it, 20 secs later 5 more ships jump in. Hyperion/Drake/Drake/Ishtar and some other one... I keep attacking the deimos hell am not giving up... I get him down to almost structure and out of no where he is gone am like omg I forgot to scram him.... fast forward few mins later pop goes me and my pod... O well :(.

I know a guy who was ratting in his Myrmidon when he got jumped by another guy in a Domi-fit Vagabond. The Vagabond got into web range and the Myrmidon's drones tore it up. But the kicker is that the Myrmidon didn't even have a warp jammer. The Vagabond could have warped off at any time. Then, to add insult to injury, it turns out that the Vagabond pilot breaking his alliance rule against piracy and ended up getting booted. I'm thinking they're better off without him.

An FC who shall remain namesless managed to gang warp half our gang to a random belt after his ship got splattered during a roughly equal numbers fight with some Russians...
Happy ending though, they landed smack bang on a 'geddon which they splattered (YARR) and got back to us just in time to see us mopping up the rest of the bad guys.

Going 1v3 in my megathron vs. a vaga, pilgrim, and lachesis. I kill the vaga and pilgrim, but by the time i'm on the lachesis I'm out of cap. For some brilliant reason I forget that I have a cap booster and proceed not to use it, and then die...

forgetting to cycle cap boosters. attacked a 3 v1 in the domi.. domi v domi, myrm, cerb. Wacked on cap boosters to help me reps/neuts, forgot to wack off. kill the myrm, both domis r down to low armour go to hit cap boosters.

For me it would have to be warping into a mission in my Cerberus with my remote repping Raven backup and forgetting to switch my mods on.
