See a manticore 100km away stuck in my dictor bubble, MWD upto it, kill the MWD in order to drop my sig, slaughter it.
Zealot warps in, set orbit, forget to put MWD on, get abused as i didn't trans it.

Missed my "golden kill" which im still pretty torn up about.
Solo hunting in my Sabre, i come into a system and lock down an ishtar.
Warp too his belt, bubble him, have a little tussle.
He gets me too half armor, i mwd away, come back in with a bit of shield, finish off the core of his drones and we're back into it.
Get him down to pretty much 0 drones left and half armor... he warps off.
Forgot to click the scram again in my excitement and he'd moved out of my bubble >:(
Was/am pretty gutted, would of been a classy killmail.

My first ever real PvP.
I was in my blackbird and had just joined RKK. This was long before the days of BoB and there is prolly a bit nostalgia here for the old guys.
Myself and Sun'ra were chasing Fist from the Jokers in the pureblind area.
Sun'ra said he had him on a gate scrambled and webbed. I came out of warp about 20km from Fist. Locked and started to approach.
When lock was achieved I opened up with my torpedos, (nostalgia annoyone) yes I said torpedos. My heart pounding in my chest and Fists ship exploded. My elation at my first PvP kill was interupted by Sun'ra laughing his head off.
When I finally come to my senses I was sitting in our homebase at torrinos no blackbird to be seen. I was saying "WTF". Sun'ra pointed out that I hadn;t hit orbit and bounced off of Fist. My torpedo blast radius combined with Sun'Ra's had destroyed my own ship and also podded myself and Fist.
Much giggling later Sun'ra sent me a pic showing Fist's and my own corpse lying next to each other. :)
Ahhhh the good old days.

The last and biggest one was in Orfrold recently.
Trying our ships in a SS with a corpmate, my megathron vs his myrmiddon.
Notice local entries, but don't feel very worried. Notice a cover ops but don't feel anxious too. Notice lot of -10 but i'm in a SS and i don't see probe in space.
Never understimate anyone. And Never ignore your instinct, this kind of alert in your head trying to tell U that U are in danger 
Then Suddenly, a domi appears blinky red on the overview (wtf), adrenaline rush my brain, and after it was a carebears carnage. Just after the domi, i remember a machariel and a mega and a t2 cruiser. I don't have time to align and warp cause point on me, so engage full dps on the machariel, but my armor was going really fast and just before i blow, i notice my hardeners wasn't on. I don't think put them on change the end, but at least i would have resist better than a cruiser .
Thx BloodCorsair's it was a nice fight and lesson  .
For the moment i'm but i train to  soon.
In every fight, mistake happend, i hope i'm not the only one.

Gate camping and scram the gate instead of the target and the target gets away and the whole gang is yelling at me lol.
Thought a cerb was a caracal and got my drake owned.
Thought i was in .4 and attacked on a gate, but realised i pressed autopilot by accident and didnt know i jumped to the next .5
Went out to pirate, got in a fight and forgot ammo and drones lmfao.

Went out to hunt.
Went to drag a cap booster from my can to the hold.
Opened the can, only to notice I had put the wrong can from my hanger into my hold and instead of 12 cap booster 800s, I have a trajectory analysis skill book, a shield recharge implant, and two auxiliary nano pumps Embarassed

This. Only on Monday I'd was just finishing off a Cerb in my Drake, when his gangmate's/alt's Deimos warped in right on top of me. I decided to continue reloading from Torrent to Fulmination before tackling him. He warped straight back out. 
The really annoying thing was that that same Deimos had escaped me from just 20 minutes earlier because I'd screwed up my rangekeeping and let him get out of web range.

Had managed to get aggro from a mission running nub corp macro raven (Called something like fdghgdgrr), by stealing from it after probing down the mission.
I went back for my vagabond and saw it warp out the second I came in... So I follow him through the gate and catch him on the other side. And there comes the screw up: I forget to turn off the MWD after catching him and I zoom in on his ship to watch the battle there. The result is that I suddently lose all speed and gets hit by 5 salvos of 6 cruise missiles. The result is me with 80% armor. I still hadnt realized that I had been running MWD speed all along, so I hit MWD button just to see the out of power message, followed by ship is out of control as the next wave hits.

Picture the scene. New to eve, desperate for that first PVP kill, i see a war target at a gate. I can easily pop him. Hes slow jumping, i lock him, warp scram him, and get owned by concord.
I had accidently warp scrammed the stargate.

That's actually pretty tough. I've made a lot of stupid mistakes. Picking up a hurricane and hulk in a belt on scan, dropping out of warp right on top of them, ganking the hulk and only then realizing I was in a 0.5 and getting CONCORD'd is up there. Along with accidentally jumping my flagged gang into highsec while pursuing some targets that had eluded us for 5 jumps, fitting an AB instead of a MWD, dropping solo out of warp into a mining op 30 km off, and then realizing my mistake as I watched them warp out as my plated ship putted around at a couple hundred m/s, accidentally insta-popping a corp member's pimped out crow he just invested every penny in, and shooting a gang mate's wreck in highsec because I already had it locked (which is apparently a no-no with CONCORD).
The best thing outlaw status ever did for me is get me the hell away from highsec.

Got into a shiney new rapier, found some noob in a belt in a hurricane, uncloaked 20km from him, scram + 2 webs on him, seemed to be going well at first, was taking a bit of damage but his armour started to drop fast, noticed my shields going down but thought yea he'll die as i enter half armour i think i'll hang around, needless to say i got wtfpwned as soon as my shields dropped coz he was using barrage, he survived in about 5% hull. Spent the rest of the day hanging my head in shame, docked up, and to make it worse, the covops cloak survived.

Speaking of MWDs I once put a 1mn into a med-sized ship then wondering why neutrons fitted. Needless to say I found out why when I was in a belt 30km off a target... .

Crow engages my Stabber on a gate. flew towards him and activated my neut II. obviously he couldnt outrun me anymore. switched to emp and started hammering down his shield and armor. at some point before i deactivated neut to spare some cap. then most of my concentration went on his health bar... was feeling good cause he was down to like 20% structure when i suddenly realize he's 20k off... obviously i couldnt catch him anymore. lol

a) Not buying the filthiest redhead I ever met the house she wanted.

b) I think I once forgot to switch some hardners on one time also. Lost a Thorax.
Had I not done a) then I would not have ever started Eve and would not have done b). So technically it was a)

had some isk to spend after liberating 150 mill from a noob with an abaddon to sell , fitted out a manticore with best named and slaped a couple missle velocity rigs on it, went to jan (i think) saw a cruser in a belt warp at 100km manage to pop him, warp to gate to jupm...POP...85 mill gone...it was good fun though
