Im not a rat but pirathunters make stupid mistakes too.
I once warped in a zombie gate camping, ( mistake 1 ) then the asked me to pay 30 mil or i lose the scorp.
I tried to run away and it worked, but.......... i had ap on and i warped right back into there trap ( mistake 2 )
So i told them that i didnt wanted to run and that they could shoot me ( still got honour) they loved me for that and i got away ..... in my pod

Heh. This just happened last week.
I normally fight in a Breacher, which isn't known for its tremendous cap. When traveling I keep the MWD on manual, as it will get me quickly to a gate without using more than one pulse of cap. When I got my Ares, I kept up with this out of force of habit.
So, I'm cruising along in said Ares when I spy a pair of pirate Kestrels, including one piloted by a well-known villain with a large bounty. "Ooh!", I think, "this Ares should be able to eat those Kestrels for lunch!". So I warp in and start my attack run. It was lovely. Cruising in at 3.5km/s, firing cruise missiles, everything looks good. I start to double back when I notice my speed dropping suddenly and I remember that I'd forgotten to switch the MWD back to autorepeat...
By the time I'd hit the MWD again the nearer Kestrel had webbed me with predictable results from a big pile of oncoming cruise missiles...
To add insult to injury, I'd gotten Big Bounty's Kestrel on that run, but hadn't had a chance to get the pod.

I was talking smack to Tank CEO in Orv since I new that if he shot at me the guns would help me kill him. Well he is oiously aware of this and decided not to go any farther than to lock me.
Once his friends arive they all jump into PF- and I sit there for a few. Well about 3 minutes later I decide, wth, they must be gone by now. Well, I used ap to jump through gate (big mistake) so when I was through I automatically started warping which decloaked my ship. Since I wasn't able to properly get ready for a warp with my MWD I was dealt with in an orderly fashion. At least they only got my Raven, and I have yet to use a clone.

attacking bob whit a celestis as i belived he was flying a mammoth, got a real chock when they mammoth started to shot cruise missiles at me!, turned out that the mammoth really was a tempest
however im making stuiped things all the time, like today i tried to attack 3 CA battleships solo whit a BB loaded whit valuable cargo, example several bp's. needless to say- that was pretty stupied. :\

believing my auto scanners information as bible (it had never lied to me before.....)and warp scrambling a c0w indy in Amarr on a 6 gun gate.....
i had assumed, without checking obviously, that since it had orange flagged his ship, (my corp war setting) that he was infact from a corporation at war with us...
needless to say my tempest lasted all of about 2 seconds and the c0w indy got alittle something extra for his journey.....

3 of us went to golgothan fields to hunt some pirates that disturbed someone, and i was in a bb - i had warp scrambled the pirate, target jammed, and webbed him...for about 5 seconds when i realized i hadnt set autorepeat on the warp scramble or web and he got away...

less then a week ago i had been playing with my set up alot as of late
so any way in the morn i spent some time staring at fitting screen swaping setups just moving things around...ive been doing this too much as of late i think
went to work
came home log into vent heard there was a ebil cow 1 jump away but no scramblers fielded ...i had some fitted so i go to the system ebil cow is engaged at a gate i warp to them slap warp disrupter on mwd up slap warp scrambler on him so he is now -3 warp core str toggel over to highs and 4 of my 6 turrents arnt even loaded
after he lost his ship and i rechecked my fittings i found i didnt even have the guns i wanted fitted on

i pop into my local hot system and found 1 corp member in a frig sitting next to 4 bs at a gate
checking info the all seemed to be ex members of a corp/aliance we have a nap with
wondering what the deal was watching local chat i uncloaked...must be a sub corp maybe or something
one locks me and opens up...hmm wtf maybe a mistake his mates will tell him were cool..2 secs later im locked down jammed and all 4 opened up on my tempest
not sure what happended to the frig guy ill have to ask tomarrow
it didnt last long first bs lost oh well i had it a long time
seems it a pirate corp that doesnt pirate pa
note to self dead weight gets moved to -10 and kos list
hmm so thats 3 off the top of my head

I chased someone across 6 systems, got them warp scrambled, webified, and then realised my guns weren't loaded and they crawled out of the system at 30m/s before I could load up :/

A friend of mine, sir tristan, left Eve and gave me a Scorpion and 20M isk. Only problem was he left it in the middle of fountain - a round trip of 90+ jumps.
Deciding it was well-worth travelling for, I paid for a pass and made the journey to get it.
I got back to Empire space and saw a couple of STK-Scientific pilots in local.
I'd been setup in my Scorpion for engaging multiple enemies at long-range (sensor damps, tracking cpu, multiple sensor boosters etc) and warped to the Yulai gate in Luminaire....at 15km 
Needless to say archangel sean's Megathron chewed up my Scorpion but, still, I got him halfway through his armour before the addiction of an Apoc/Megathron finished me off.

i undocked to find an enemy Apoc warping in, thought, yeeeeeeeeeeeeeeeeeeessss, lets get it on, fired up my sensor boosters, turned on my hardeners, hitting warp scrambllllleeeeeeeee, WTF, i warp away to a gate.
when i undocked i had set AP on, but totally forgot about it  ..
warped back to the station, and he was gone.
he seen the funny side of it though.
looked like i had ran like a wee girly..

Way back when most people were only in cruisers and the first battleships were being built, and before projectiles and mod stacking was fixed... I heard in Obe local that Lord Zap was camping a gate in a Dominix. So I decided to warp to the gate in my thorax with an insta bookmark. I wanted to see how much damage his hits were, and then I'd just jump through the gate. But after he locked me and fired one volley, all of my shields and armor and structure was gone, and when I tried to jump it said my ship was out of control, then my ship blew up and my pod went with it.

Just recently I was involved in a skirmish in empire where I was faced with 2 enemy pilots at a gate. knowing I had reinforcements close by and a "goodly" number for jamming via multi's I decided to take em on. Unfortunately I had forgotten to turn off the auto-target back feature, (had been npc hunting) so an allie of thiers whom i was not at war with had targetted me and caused me to target him back (pretty cheesy tactic, but still my own fault for being careless). Assuming my extra targets where just drones (and just a bit excited) I cycled him right in to be jammed and promptly got wasted by 6 very ****ed off sentry guns.

When castor came in and m0o had started hitting CFS, camping Gehi system, i jumped in to find several members at the gate, they all target me, no one fires.
Bursting for the loo i click, or at least i tought i had clicked dock at station.
I return to find myself scorpless and in a pod. I had infact clicked on warp at 15km, nice easy target for them, lol.

I felt like fighting so I took my apoc out of Gehi station and got a m0o guy to come to a planet. He came over as did his buddy (tempest+mega).
I was about 150 km away and flew around waiting for Araviel to set up a bb and come slaughter them with me. So Araviel got ready, the tempest warped out, I approached the mega.. when about 60 km away the tempest comes back in and I still have my mwd going toward the mega, I tell Araviel to warp in with her Magnetometric jammers and disable the mega.. I go boom before she warps in since I left my mwd on too long And didn't have cap for my XL clarity ward.
Araviel warps in just as I go boom and loses her ship too to fof missiles from the mega..
As if Apoc+bb vs temp+mega isn't stupid enough, I foolishly didn't tell her to warp to a moon nearby and align for warp to me first before engaging 
Naturally I congratulated them on defeating us and went and picked up my gold magnate to continue fighting m0o.
Almost lost it too to 5 m0o frigs that chased me around for about an hour..

Back when sentry gun ranges were at 55km, I was sat out of range at a gate in aurohunen, in a BB, looking for passing ebil doers. I had seen a pirate passing through a couple of times, but he had below -5 sec. So on one of his passing journeys I desided to fire a cruise missle at him. I new it wasnt likely to hit him as he was in a frig and I was miles away, but what the hell I was out of sentry gun range.
But I had been drinking, and most people know I do st00pid things at the best of times. So being the genius that I am, like 30 seconds later I decided to warp out, and then warp back in within sentry gun range. Bye bye meh BB and f-90 sensor booster I am teh daft
That is just one of the millions of times that I been killed by the vicious, and hatefull beings, known as sentry guns 0 d00m.

Lucky me  it happened on my 2 day in game when i still had an Imparior (rookie ship) so it gave me a free one. haha he wasted 2mil on my ship with the 2 large emp smartbombs. He really didn't even mean to hit me and he felt sorry so he gave me 100k. that money really helped me get on my feet.

I tried to take on 3 pirates at once....alone...in space....where no one can hear you scream!
Apoc and a few choice mods later....
Although Kudos to them that I fought...they let my Escape Pod go through undamaged a few minutes later...they said something about over-sized baloobas.

Well my biggest mistake was reinstalling EVE than hoping into my tempest undocking from the station in 0.0 set autorepeat on my missle bays but forgot to on my hardners and shields.
Also i forgot about when u fire your guns a box appears on the screen saying you made an agression action that was annoying when 8 boxes appeared all over the screen blocking my view.
Entered battle and got ganked
Lost ship of course such a nub i am , i am still hateing myself to this day as i was far more powerful than the megathron that killed me:(

Um smartbombs are not wasted when used.
