After waiting for several days our corp rep got this response to his petition for reimbursement.Please take note that we did not find this post in our searches of the forums since the title has nothing to do with refineries. also note that we have lost 162 pieces of ice.

From what I have seen, the refining operations appear to fail under the specific condition of: The pilot who starts the job is online, AND is more than 1500m from the refining array at the time the operation should finish.
In this case, the refinery becomes inaccessible until the following downtime. After downtime, the items that should have been delivered by the job will appear in the array and the refinery will work normally again.
If you wish to have successful completion of refinery jobs, there are 2 workarounds - At the time the refining job should be finishing, the pilot who started the process should either: a) Be within 1500m of the refining array. b) Be offline."
Unfortunately, I am afraid to say the lost mineral/Ice quantities, if not already recovered during the next Downtime, will be lost since our logs remain unfortunately limited in this circumstance.

How about this as a solution. I'll pay you 15 dollars American every month, and you believe me when I say my corp lost 162 imaginary pieces of ice that took 4 man hours to accumulate to a program that CCP programmers, that are paid by us, the players, broke in the first place.
