a) A good fleet commander. A toss up between FC and teamwork players. Both are important, but I think that a mediocre fleet with an excellent FC will fare better than team players with some horrible decision making.
c) Accurate reconnisance. Really knowledge about your surrounding is key. From carebearing cloaking Raven to hardcore Nano fliers. They don't die because their recon is good. Leeroying into a system is bad.
5. Assumption: This happends in low sec . A NPC newb would die instantly in 0.0 under those conditions.
The NPC is probably an alt of either the command ship (CS) or one of his friends. If you expect him not to be stupid he is out to kill you, so he is now working as bait for his mates. Now since there doesn't seem to be a gang on the other side of the gate trying to see if you can break it wouldn't be a bad idea, but be prepared to flee. And for god sakes, don't trap your own gang in the bubble 
6. I do not like the way the question put up. The pure gankage nor the pure nano only fits exists, despite what the forum here tries to tell. While they have a basic way of fitting they often have spare high and midslots available normally featuring various kinds of E-War, not to mention the use of EAS and Recons in both gangs kinds.
c) A balance of lots of different kinds of ships.

Guess homogeneous surround the gate with your cruisers at up to 60-80 km and create a net, if people have webbers that is. I wouldn't really recommend it since said nanogang will completely destroy your gang since you are too scattered to help each other. But two vagas would be worth it.
Other option: People fear dictor/hictor is high so the nanogang will want to kill it first, so maybe having it place a bit out with some webbing cruisers could lure them. tricky though.
8. Kill it. Really, there shouldn't be reds in the system to begin with
9. The nano gang fight is a perfect example of why you situation demand you to switch target often. The normal approach (from those I have that knows to pvp imo) tend to hover around your fleet hundreds of km away and then make small strikes at the fleet slowly removing ships that could endanger the nano gang. With ship constantly go out and in of targetting range primary must change often. In a tank'n'gank brawl focus fire is the key. If you cycle targets some will be able to actively tank a lot of damage making their fleet go down slower than focusing. Although smaller ship may have advantage of web+scram various important targets before they get called.
10. Dessies main use lies in killing small ships not laying damage on primary. Kill the cepters since you are in one of the few ships capable of it.
11. Odds are already stacked against you. ECM to take out the BS'es and several ships excellent at taking out fast ships. The three hostile BS'es have enough firpower to get the a BS down. Huggin, blackbird, blackbird, Scorpion, Dominix, Apocalypse, Armageddon, Thorax, Vagabond, Crow, Muninn

Since my kill was a pod from a carrier that I helped bumped of in my trusted Rifter, get into the best frig of your race (BKG, M.Pire, CoW, F4t4l etc vs Razor, Smashkill, Triumvirate, droner IIRC). With 2 million you are not going to top damage in a fleet fight, but the difference between a point on a BS is the reason that it dies or not. During the BKG fight several battleship tried desperately to target my rifter and send heavy drones after me. Ahh good times.
13. 40 Dessies  Aren't they almost able to alpha a BS? In that case it will be dessies. They can also warp in and out of battle if targetted.
