Getting scammed out of 100 mil after I made a point of making sure the guy wasn't a scammer... I sent a TON of hate mail that day.

Flying a Prophecy into a lvl3 mission and realising way too late that those red crosses on the overview look a bit big to be cruisers. Checking again while I returned in my pod to find I was talking to a lvl4 agent instead of a lvl3. Scratch one rigged and T2 fitted Prophecy.

As a newbie falling asleep while in 0.0 and getting killed and podded - 50 mil gone
Buying GTCs from a guy, turned out it was a ISK farmer scamming with codes - about 2 billion gone
Falling for the regular ship selling as navy ship scam - 150 million gone (at least i didn't buy it out)
Various mistakes done while trading or with contracts - 20-100 million each amounting to another bil probably
In my defense all i can say is that i made way more ISK on mistakes of others than i lost with my own mistakes

I've warp scrambled so many gates in empire it's not funny anymore. It's hard to be a bad ass when you can't even target the red blinky thing.

I bought a couple of launchers for my new, and first, Raven in a .4 system. Then I flew to that system in my unfitted Raven, stuffed the new launchers into the cargo hold, along with other equipment I picked up along the way, and headed to the closest gate leading to a high sec system....using autopilot. As soon as I got to the gate I was locked by a Megathron and what did I do? That's right I panicked and logged out. Thank you om'gawd for sparing my pod. 
Of course not quite feeling stupid enough yet. a week later I buy a new Raven, and a couple more launchers in the same system. Off I went in my new unfitted Raven to pick the stuff up. This time I was smart though. Instead of putting it all in my cargo hold, I fitted it to my ship. Well when I got to the same gate, using autopilot of course, I was locked by the same guy. Oh well, at least I fired back this time....I just know that if my second round of missiles from my 3 launchers hit him, that I woulda podded him.

Attacking a POS in low-sec Amarr space after seeing it only had one defence turret and a guy inside the shield who was "afk". I lost a 50-60Mil Myrm

Yeah... just started playing again, saw the new contract console, thought "Wow! Now I can SEE what they are selling! scammers are screwed now!" And bought ONE piece of salvage that was labeled as 500ish... poof, 40 mil gone.
And I used to warn all the noobs in chat to LOOK at what was being sold...
Heh. Remade the isk in a couple hours, but that was DAMN annoying.

Got my standings up to the point where I could do level 3 missions, got a drake, realized that at 2m sp it was still rather hard to do those missions, got 3 core defence field purgers with the 36 million isk I had left...
...and then put the rigs on my salvaging cormorant, instead of the drake.
Since that just happened an hour ago, right now, I'm banging my head against the wall and preparing for a two week grind to get enough isk to get new rigs

I can't recall any really big newb mistakes i've ever made. I've made some expensive ones, but it's usually the small things that get me killed. No single really bad thing(s)

Spending months in 0.0 then cj'ing to Empire for a break. Went into a friends mission and was jacking around and targeted a can of his. Concordokened, 30 mil loss.

Sure, I have loads of these.
1. Got my neutral corp alt flattened by an xDeathx Alliance member who I had previously talked to on main (who is dark blue with) and told them my route, what I was carrying, what my fittings were etc and not to shoot me and maybe provide escort as noone else was around... Got killed and looted by him and a friend for about 400 mill that day.
Sent tons of hatemail for a bit, hope you go to hell, stuff like that. Quit eve for few months, came back carried on as usual. :)
2. Alliance got beaten to death, station lost, never understood how mechanics worked of station takeovers, around another 300 million of unpackagable assets which had to be desolved.
3. Lost a faction fitted marauder one late night when I was real tired, got really lazy in my mission strategy, set too many spawns off early, managed to just break my tank, and I died before I could kill the 6 or 7 frigates scrambling me.
4a. Attacked some guy in lowsec in a belt, in a punisher frigate with afterburner and scrambler, guy microwarped away and escaped just as he took hull damage.
4b. Warped to gate to see what happens when criminally flagged. Nothing fires at me, cool I thought... I open map to plan my route to nearest place to buy a webber only for the gate guns to decide to open fire just at that point.
5. Me and three friends attacking a -10 sec nano vagabond at a gate all in tech 1 cruisers, on taking too much damage I had to microwarp away from his drones, friends were taking damager to from his main ship, so we all head to the gate. Just as we get there, I go to jump... The stargate denies you permission to jump for your recent acts of agression. *mashes key* The stargate denies you permis... The star....The star...??? "WHY, he was -10 and flashy red!?!?!" I cry, at a time when I didn't understand the game mechanics very well. *drones behind me catch up *BOOM** Needless to say we all died to the vagabond.
