OK...I've been playing Doom3 alot and the whole game is pretty creepy, but it got me thinking about individual game levels that stand out in your memory as being ultra creepy.
I'm sure there are lots of great contenders here, but if you like to be scared here is a game level I would like to recommend to everyone.
Thief - Deadly Shadows...the Orphanage level.
Without giving anything away...This level just oozes forboding atmosphere. It is by far, the scariest level I have ever experienced in a game so far. Not because of a heavy smattering of blood and guts, but due to some truly disturbing subject material and how it's presented to you. Amazing job by the game developers in creating such a feeling in a game level. By the end of it, you'll be glad it's over and you made it out, so you can give your heart a rest.
Anyone here who played this game through can attest to this amazing level. Anyone here that is a real gamer, you owe it to yourself to try out not only this great game, but to experience such a "stroke of genious" level.

I remember the Entertainment deck on System Shock 2 having a particlarily scary atmosphere.

Max Payne - Valkyrie Hallucination Level
Walking down trails of blood, reliving the main characters nightmare past of having his wife and child executed by drug users. Hearing the baby whine in all manner of wierd sound effects plus the gunshots and freaky slow mo effects...
Seeing the child's toys move on their own and the whole freaky thing inwind in green hues of terror, perhaps not as plain scary as other games but down-right head screwy freaky nonetheless  --------------------------------------------------------------

Max Payne - Valkyrie Hallucination Level
Walking down trails of blood, reliving the main characters nightmare past of having his wife and child executed by drug users. Hearing the baby whine in all manner of wierd sound effects plus the gunshots and freaky slow mo effects...

Playing "The Suffering", with lights off, late at night using headphones is a truly, in my opinion, terrifying experience. I have never been as jumpy playing a game before this one came along.

I agree 100% with you there Mr Binary. Thief 3 is one of the few non MMO games that I have played right through recently and that level was very, very creepy.
Without giving too much away I remember going to see who was at the front door as I was approaching that room in the orphanage attic for the 1st time........

I agree 100% with you there Mr Binary. Thief 3 is one of the few non MMO games that I have played right through recently and that level was very, very creepy.
Without giving too much away I remember going to see who was at the front door as I was approaching that room in the orphanage attic for the 1st time........

Aliens versus Predator 2, on Hardcore of course, when you have to go into the sewer system as a marine and the aliens are coming at you in the dark. Or for that, any moment in the first one where you meet a facehugger or... (you get the idea, I love AvP)

i think the origional AVP was scarier than AVP2
and any indoor-underground-industrial level in half-life with headcrabs

any part of doom 3 is bloody scary 
Im only upto the communication tower bit atm, but some of the levels have been great, especially the ones where u are in pitch darkness trying to kill a zombie, u think u have killed him so you turn on your flashlight only to have him standing right up close infront of you.

AvP 1.
Hands down, most disturbing game I've played.
Course, I did my best to make matters worse.
Headphones. Home alone. Lights out.
I lasted about 2 minutes post first alien (playing as marine), then I suicided on intention, and quit game.
Jesus. That was scary.
And yeah, AvP 2 wasn't nearly as scary.
(sidenote: Anyone recall the soundtrack for the game Blood? That is possibly one of the most disturbing soundtracks I've ever heard. Used it a lot for some Call of Cthulhu ;))

Not really that scary, but I spent some tie in the tree houses on Farcry when I first came across the mutants. Things running at me with claws when I wasn't really expecting them made me jump.
