- Once, I mistook a normal ECM mod for an ECM Burst. In Motsu. Yeah... 
- More than once I've forgotten to accept a mission before undocking, then going to said destination and having to fly back to accept it.
- Setting destination to a chain of systems in high-sec, end up jumping into Nalvula instead with a hauler containing everything I had. Luckily it wasn't camped... 
- Forgetting I had a POS set to shoot neuts, I warped my scout alt to it for a few minutes. Needless to say the bright light and subsequent *pop* was..../facepalm

0.5 belt + shiny new Worm + AFK = Worm wreck.
Bought a "ferox" Cormorant for 8m in a direct trade back in my newbie days.
My first ever 0.0 alliance op - Lagged behind the fleet because I didn't know what I was doing for the most part. Enemy fleet intersected our fleet's escape route (This was after a failed station siege). I arrive 15km off a gate and notice the gate is flickering on and off like a strobe light at a Darude concert. I've never seen so much red...
Tried to take on a Harpy in my Kestrel during my first ever empire war.
Made some friends and covered one of their mining ops in Seyllin with my shiny new Armageddon. Said Armageddon was fitted with medium lasers. I just so happened to have a negative sec status at the time, so this guy in an Apoc and his friend in a Thrasher think I'm a pirate and drop in on top of me. There went that Arma. (Moral of the story: Don't fly it if you can't even fit the right guns).

Trying to control the "frozen food" marked. :D I did spent allot of isk, + 20h Before I gave up.
- The reason: It was a mission that had ".....get amount of froze food...... "
So, in every station, I got tons of it, etc.. Until I found out its "impossible" to control the region,
due to the numbers of stations, and isk.
- I had to buy all the frozen food the at the station, and set an higher price = Fail
In short: I did spend all my money, and had frozen food up to my teeth..
Then spending 20h+ H trying to get some isk back, it was hell... :D

Lost a DS fitted Tengu to 6 cruiser rats. I was hacking and it took a lot of time so i started to browse around with deactivated Booster, cause my cap wasnt stable with the hacking module active.

Feeling slightly less stupid/alone now after reading some of these.
Trying to play eve at work, someone came into my office. Alt/tab out. Alt/tab back in a few mins later to see my DS fit paladin a smoking wreck. ooops.
Day 2 exactly the same situation but less DS mods.
6 bil in 2 days.
Who said lvl 4s were risk free?

warp into sniper position to ping at 6 hostiles
align to point 'A'
stay around long enough to let the cloaky one get up next to you
press warp to point 'B'
*poof*
go afk in a gang at a pos, get gang warped out, *poof* -ship and pod.
ignore the neutral in local, there are enough blues they won't attack me.
jump into highsec with gcc.
jump into highsec with low sec stauts, ah well now you can just warp off, or back in the day you could just approach the gate and tank it, and if lucky get a friendly bump.
listen to techzer0 and jump into a fight that is like 3v2 + 5 others in local. and not notice that your third is a highly skills gallente bs pilot but is flying a drake at the moment.
go afk when shooting at the ship that will trigger the next spawn, with half the tank going.
overload and burn out all guns, ah well at least I didn't lose a ship from that.
listen to someone you know isn't one of the best pilots, and warp in to try to help them, to find out they gave very very wrong intel on what was going on.
hang out when things are going bad, or about to go bad, when you could have warped out
