what's you best contribution?
some of mine:
Dominix - of some stupid reason i feelt i hade to wait for the drones to return before warping out when i was down to hull(witch was already damaged from the beggining)
Apoc - some months ago, did get my eyes stuck on a ****movie and didn't realize my armor rep was on "manual on" when hunting some 0.0 rats

Oh yeah, just my kind of thread.
I once lost a raven several months ago when I let off six cruise missles at a frigate. The frigate was outside a station in empire and he was helping a corp mate test his ship setup. I didn't realize this at that time, and to me, I thought it was just one player bullying the other. I didn't realize at that time that corp mates can fire on each other without fear of retribution.
I hadn't had any pvp experience up to that point, so the whole thing took me by surprize.
Sad thing is that if I wasn't so damned drunk that night, I could have tanked concord all the way to docking range.
Kids, don't drink and fly...at least not while you're a noob.

Well, i was camping in Y-2ANO system in my new Stabber with 280 IIs. So after some time i had nice killmails in my inbox, so i thought its time to post em. Local was clear for like 10-15mins already, so i, warp to ZXB gate at 100 and alt-tab to the FA kill-boards and start posting. So once i alt-tab back i see 3 intys approaching me and already tackling. Well, i had no tank, so it was only matter of time 
But that was so dumb to go afk at gate.....still dunno why i decided to alt-tab at gate and go post killmails

I was heading to a 0.0 system with 2 corpmates and one of them was scouting. The scout quicly said "clear" as he jumped into the system, before he let the screen load completely reveal the 10 pirats camping the gate :)

Got my first zealot. This was after a whole month of waiting in line for NAGA to deliver. Took it out after fitting on a lvl 3 mission. Kind of went semi afk, came back and was in a pod. Realised I had forgotten to turn on my armour repairer on!

I got 2 good ones. 1: Was in a crusader doing level 3 mission back when an oversized AB crusader could destroy anything short of a BS with a good tank. So I warp into the mission and since it was a long warp I quickly run out to my porch to check on the steaks I have cooking on the grill. Soon as I leave my seat my fiance jumps on the computer alt-tabs and jumps on the internet to check his mail. I of course by the time I get back inside had completely forgotten about warping in to the kill mission. By the time I get my comp back about 25 minutes later I'm in a pod looking at my can with sengrin min (sP?) laughing at me. 2: Jump into a level 4 guristas plex with my zealot, I see a horrendous blob of roughly 25 cruisers and 30 frigs staring me in the face. Undaunted I shoot down one of the frigs, it melts before my first cycle even finishes firing, so the mod doesn't aggro. I then target a cruiser and unlease on it. The entire blob locks me at about 30km, and my comp lags hard. I can't even activate my armor rep through the lag, let alone try to warp out. I was toast in about 30 seconds, if that.

today I forgot about my tank during a mining op. I thought it was a sustainable tank but I guess it wasn't 2 hours into the mining op I see a big flash of light. Not good not good at all

once before dt, jsut woke up and logged in to c for some bypassers in fd-mlj - pf-346 area. so i undock in ceptor fd and jump to pf...killed some frig...
for couple of mins it was all clear so i decided to go back to fd...aproach gate, jump...
im cloacked i c griffin and some other frig at gate, cant remember which one now. decided to go for griffin first. lock, orbit, web, scramble, fire, my first salvo goes out then suddenly my shields gone, in next half sec armor gone and then ship under control. i was like wtf that other frig has cuz griffin didnt even locked me back...
**** it lets save the pod, right click to chose a ss, top bm says orvolle insta dock 1st station, wtf why r my orvolle bms visible in FD O.o . i just choose some other ss and warp out. very surprised about all that and bit ****ed how 2 frigs easily killed me...but still my brain wasnt clear enough to think about everything...
so i decide to check the log to try to find out how they blow me up so fast... wtf r sentry guns doing in fd ? O.o
omg im in orvolle ¼_¼
just wrote big lol in local and dt came :P

Not my own death, but it happened on an op:
Non war target in a 0.5, someone goes after it, loses ship to concord (was in a battleship). He warps to a safespot grouping. Lovely pod bug flashing, one of the tacklers gets a little triggerhappy and kills the pod, then loses his ship to concord shortly after.
Sorry just remembered it.

Megathron fully fitted with tech2, 5 modules were named rest were tech2. I logged off the previous night in a belt cause I was lazy (was npcing).
Next day I log in thinking I'm at my safespot, I see the character thing loading so I minimize the eve window and read the forums about 30min, by that time I had forgotten I had logged in 30min before. So I pull up the eve window and...
There I was in a pod. Found out that a cruiser spawn blew my ship up 5min before I stopped looking at the forum. Lost about 80 million isk

Well, i fly my typhoon into obvious traps and outnumbered situations just for fun. Not a mistake but stupid, and fun.
Um, I forget to turn off my sheild booster now and then when killing npcs. i lost a tempest that way when my cap drained and i thought i could take the spawn still. Turns out i couldn't get into warp fast enough before i lost my armor and hull lol.
I lost another bs when running dual clients and i alt tabbed into my alt as it warped to a POS. both clients lagged and before i could recover i was in a pod on my main lol.

Was flying around PF-346 in my Crow NPCing when a dude in a harpy warps in to belt and starts approaching from about 70km out. So i just keep current range on him til some other peeps in local com to kill him. But i decide to mess with him and move in and orbit at about 50km so he cant kill me but get really ****ed off. Apparently i hit the "Approach" button instead of "orbit" and get pwnd by his tech 2 rails before i can fly away :(

For some reason every time an engagement could or is going to take place, I always get the idea that I am some hero who is going to swoop in and triumph over evil some how and save the day. When in actuality it just ends up being me, in my Scorpion, getting owned by 7 or 8 guys in battleships and other various elements, who divy out my loot and laugh in local at why I took on 8 guys.
And the whole time I am sure I was thinking...
"8 war targets? Haha, I'll be fine."

Killed my first Domination BS in my Apoc and was feeling quite good about myself. I then combined this with something good to drink, either vodka or bourbon, and went on to look for more fun stuff to kill.
In the next system over I came across a BS and engaged. A few minutes later I realized I hadn't turned on any hardeners or reps. Apoc go BOOM.

Lost a Cerberus about a week ago because I was playing EVE, camping a spawn, and listening to music (thus I had EVE sound off and my music cranked up). Just as I finished killing off the spawn, I got IMed by a chick who I really enjoy talking to. I spent about an hour chatting with her, tabbed back into game, and was looking at my pod. The spawn had blown away my Cerberus, fitted out with all 'Arbalest' Heavies and a lot of named and Tech 2 gear.
The funny part is, this girl also plays EVE (before anyone asks, yes, I've talked to her on TS) so she felt SO bad about it, even though it really was my fault...
Bottom line is, guys, if a chick IMs you while you're playin EVE, Ctrl-Q... it's easy to forget that you're still logged in.

I was in an empire system right next to 0.0, sitting at a safespot when I logged in. I got a report from a corpmate that the 0.0 gateway was camped, so I decided I'd head back to station and dock. I think I was reading CNN or Slashdot or something at this point, and not watching what instajumps I was using..
Needless to say, I instajumped and gated into 0.0. When I went to dock, I looked at the system name as was like o_sht. My Apocalypse was quickly vaporized.
At least I got my pod out.

HA! Got you beat...was in a gang in YZ, they called align to moon blah to warp out to, I didn't hear it and warped to a different moon - which had a hostile POS at it. Compounding this was the fact that I was so suprised and disgusted with what I had just done I forgot that I had mounted enough WCS to warp out despite the warp scrambling battery - I therefore didn't try, and of course then needed another megathron.

I was going to join up with a Huzzah fleet out in 0.0. I knew there was a bunch of m0o guys between me and the fleet but I decided to go anyway. I was flying an EW scorp with no warp core stabs and nobody else with me. That's pretty stupid to start with, but I didn't care and my insurance was about to run out anyway.
This was the result.

Once i was in a 0.0 in my apocalypse and i was trying to kill a sansha mining op spawn.
Killed the BS's, cruisers and tacklers etc with ease and these was just 1x 500k sansha indy thing about 80km away.
I alt tab out to go have a look on the forums for abit while my tank is on and quickly get a drink, alt tab back in and my armor has just failed before my eyes because the 2x 500k BS's have just spawned back
Another time i lost a megathron, tried loading the map while jumping in a 0.0 system and it froze on me, just before i closed eve to relog the gate activated.
I log back in with a really bad feeling thinking i should have logged with some random alt to convo my gang m8 and ask if local is clear, i didnt so i carried on logging in.
Warped back to the gate, an armageddon and claw are there waiting for me, pop goes the mega (setup to gank) and my pod too (1st time podded, and only time)

I lost my retribution last Saturday because i was distracted. How?
I jump in 3A1P I saw 2 hostiles in local one of them a ceptor 120km from the gate.
Then I wrote my report to the chat and I was ready to bugger off. My wife came in and start whining why I didn't weight myself yet. She couldn't stop and I turn my head to see her. An Ishtar jummped at the gate and pump drones, I start hitting him (he loocked me and i was in autolock) and I saw that 50% of his shield were gones. (lost my concetration insteed of run).
After few seconds more people logged and shred me to pieces along with my pod. My wife all that time didn't stop shouting behind my head. (i didn't said a word).
And when I pop to my cloning station she said "A fine. now you are docked, go and weight yourself". 
Morale. Obey your Master. You will die either way.
Btw I saw this kill in the killboard today.

Brilliant to see some honest replies! So here are mine..
I play with a laptop sat on my sofa, which explains why...
1. Fall asleap while late night mining, 1 too many beers - Lose large barge to 0.8 rats. Twice.
2. When I first started playing, I thought if you ganged someone you can fire on them in empire. So when someone asked me what a webber was, I ganged them to show them.
3. Not me, but a new corp mate shot a station be mistake while camping some war targets. (corp title now "lemming")

After a fleet batttle, KIA retreated to a ss (having killed more than lost of course)
One of our pilots, who shall remain nameless, jumped out of his ship, to go scout the gate in his pod, unfortunatley the only thing holding his ship together was his Skills.
As he ejected, the ship went pop.
He never heard the end of it.
