Hmm. This is a neat thread..
Biggest CCP Blunders:
Balance: Level 4 agents screwing with risk vs reward, and causing a movement of players from 0.0 into empire, in a patch termed "EXODUS". Absolutely nothing beats that.
Damage Control: Undoing the accidental release of one Jovian mining laser by replacing it with a Miner 2 BPO, effectively allowing a few corps like Techell to sustain a pure monopoly for weeks and weeks on end.
Events: Gallente elections, lol.
Personal: GM taking back two battleships I had stolen from safespot just before downtime, and giving it back to their previous owner, claiming it was a bug with a previous transfer request. Resulted in both the owner and me getting an apoc and raven out of it, then the owner placing them back at the same safespot a week later, effectively allowing me to steal the same two ships twice.
Biggest Political Blunders:
Ragnar's staged coup against those who voted to support TTI dring the Evol-VA war, then allying himself with pirates like SPVD and Algazara's corp who ditched them when they stopped getting cheap ships. Resulted in the near fatal collapse of TTI, the largest and richest corp in beta and early retail.
NSA leaders making pre-emptive plans against BoB while allied to them, and being completely oblivious to the BoB spies within their alliance. Refusing terms and declaring hostilities while 95% of their pilots decided to give up within a few days, effectively dismantling one of the largest northern alliances.
Curse Alliance being the first to adopt NBSI policy, then seek alliances with FA and PA against BoB, which never materialized because of past hostilities and mistrust and the dealings being posted publically on these boards. Then blaming the subsequent military defeats to m0ovolution/BoB on the miners in their alliance which resulted in the formation of various sub-factions within the CA.
Biggest ISK Blunders:
1 trit for 100mil isk, then Roark placing a bounty on the person who refused to send it back, which resulted in even further losses for TTi as various pirates and brigands came to protect him by declaring war on TTI.
JVol buying two malkuth launchers in Orvolle the other day for 7bil isk.
Khatred losing a 30bil isk outpost construction platform to 4S Corporation, because he failed to accommodate for adequate protection.
Biggest Personal Blunders:
Fitting a cloak to my rifter during the GNW, when they were first released from the Jovian event. Scanned down a pod and started shooting at it, realizing I did not have enough ammo, calling Estarriol from Evolution to help me. Right clickeding on the pod to hit approach, only to find out it was empty, and ended up boarding it just as my rifter and estarriol's merlin finished it off. Got sent back to Gehi, some 80 jumps away from Venal - leaving a Rifter with a 150mil isk cloak sitting in space. Got my implants and clone reimbursed, and Rifter recovered, but it left me out of the war for a good week. Was the only time I lost a clone in any sort of "pvp". :)
Buying some Modal Mega Neutrons for my Blasterthron for 27mil each and undocking after a major patch, engaging an npc hunting Armageddon with no spare cap charges in my cargo, then finding out the boosts done to pulse lasers were pretty severe. Was the only time I lost a battleship in pvp, to sheer stupidity on my part, Wheya the Khanid RPer was the one that got me I believe ;)
