My vote goes to neur0n
Even though hes a great pilot hes is probably the clumsiest of all ...
Undocking in an unfitted raven with a security status of -12k and getting killed by 2 inties ...
Then undocking in a almost fitted raven to go pvp ... poor guy forgot a shield booster
Then losing a bs in a belt cause he had something more important to show on his webcam then to actually look at the game ... was quite funny seeing his reaction live as he was losing his raven ...
Losing numerous ships as he drift away from target in combat prolly cause hes too busy trying to pick up chick over msn ...
Ahhh so many good times :P

The day I got my first, and one and only Commander spawn, a Domination Angel, 4.5mil, I beat it (got a *****in' domination reflective plate, w00t!) I continued to Rat. Later I came up against a simple BS spawn. Then I forgot to turn on my hardeners and armor rep. Needless to say, I quickly lost my Apoc.
Many of you may be surprised to learn that alcohol was involved in this altercation.

Not nearly quite as bad, but 0mega traded a raptor BPO for 10 scorps i think it was. Not as bad as me and Fang afking through Yulai while at war with Zombie. . . .  I wonder where they got their idea for ganking in Yulai from? 
As for dumb ship losses . . I will not mention Gen's Navy Apoc, I won't mention Gens navy Apoc.

Wasn't there a m00-pilot (perhaps Ywev?) a long time ago how actually managed to pod himself while trying to capture a ship he had held for ransom or some such? Not the biggest blunder, but still...

After a fleet battle, a KIA pilot was warped away to safe spot. A scout was requested and said pilot volunteered, ejected from his ship, at which point his brand new Battleship went pop. Only his skills were holding it together ;)

An SA pilot once asked for gang in PF-346. (Obviously he was asking other SA'ers), but i obliged and sent him a gang invite, surprise number 1, he actually accepted it, surprise number 2, when we warped to him, he was with his entire fleet, which got spanked, surprise number 3, he was acutally flying with us asif we were his gang buddies, surprise number 4, we never even noticed he was there because we didnt have gang members on overview!!!

Once upon a time in the NVA vs TTI war one of Ragnar's merc commanders "smoked" (the ceo of M3G4 corp) was fighting against this small NVA corp called Pain Syndicate. So the fighting kicked off and smoked was getting quite baddly beaten up and he ended up ejecting from this battleship by mistake rather than warping away. His pod got targetted and he wisely ran. Taking the opportunity for a big humiliating win the Pain Syndicate sent one of their pilots back to dock and jumped out of their ship and rushed back to the unpiloted enemy battleship. Smoked had returned also so there was a pod race! Narrowly pipping the M3G4 ceo to the post the Pain Syndicate guy got into his enemies ship and blew Smoked up with his own recently vacated guns.
I'm still yet to hear a clearer definition of the word "pwned" tbh.

During our holidays in sindicate me (claw), and my mate - WRWR (arma) decided to travel around, searching for frags.
We spotted 3 bob vagabonds, they beating lone dominix. Pilot of dominix ejected, and 1 of bob guys decided to take this bs, and ejected from vaga. I asked for WR to jump, and warp on this planet (he had heavy neut + web - so he get a chance against 2 vagas). He did that, but seems, that vagas were armor tanked. So he tanked vagas for some time. At this time, i died in my claw (didn`t want to rewarp) and started to approach empty vaga in my capsule.
- Bah. WR died.
- Bah. 1 vaga started to approach me.
- "You can`t enter ship blah blah 13 seconds left"
- Omg, vaga is in 15km from me.
- Ooops, my capsule is in armor.
On the last seconds i managed to enter abanoned vaga, mwded away, and warpout. I was very glad to get this ship, as my previous vagabond got blown up few days ago. But still - WR lost his battleship, (around 120m in fitting - insurance).
When i traveled few systems back, to Orvolle, i opened cargohold, and smile : 1 cap rech t2, some cargo extenders, few tachs t2 - enough to cover loss of my mate  . So i get vaga for free. This baby is still in my hangar

Nah, that was sutty, the famous "sutty manouver"
The guy ejected and sutty boarded his ship, only to get podded by the stream of his own torpedoes, back in the days when they did splash damage.

I remember a person up north who was fighting a losing battle in his apoc.
When he realised he was dying he quickly ejected his pod from his ship but he forgot to turn the smartbombs off so he got podded by his own ship.

Ywev was the first to do it though
It was a dominix, not sure about how he managed to shoot himself though. But it ended up with someone else chasing it frantically in a Bantam while it continued on its merry journey.

The biggest blunder I've seen in a while was my own. When I forgot to refuel our pos's in D4R I effectively saved PA from certain destruction

Too many to list. But it is nice to see other people listing some of my greatest personal achievements as "blunders"!
Some notable mentions:
- Losing 40+ ships as an NVA FC against Forsaken Empire back in the day. (4 hour mexican standoff + JIP cluster**** = quick demise)
(1 million ISK to the first person who knows what a JIP is)
- Smartbombing my mates at what I thought was an exclusively Oberon mining op in empire.
- Barhoping with Robeyone.
- Stumbling accidentaly into a chatroom where Jade & DB Preacher are having a naughty round of cyber.
- Thinking roleplay in eve would ever pan out enough to be worth the effort.
Just the tip of the iceberg.

you may be speaking of my experience. i went to lock a target that was in the process of being destroyed, and instead clicked board ship. i tried to board a ship that didnt exsist, somehow died, while my scorpion continued to mwd away. 2 evol corp mates were there at the time, and were trying to bump the mwding scorpion but it wasn't doing much.
eventually, a dev named kwai came and helped fix this.
this was all captured on a teamspeak recording that i have.
