Also, I had the old blunder of going totally the wrong way on insta jumps mid-way through a journey, ended up right back where I began (Paying attention FTW) -

Well my biggest blunder has to be when I lost my brand new bs. I autopiloted a few jumps and started browsing the internet. After about 15min(if not more) I realize that there is a faint sound in the backround and I alt tab just in time to se my brand new geddon explode to 2 of those 2k gate rats you see in some systems in empire.

2 minor, but nonetheless funny blunders that I can remember.
1. While in a BS hunting party in EC-P8R, flying a cov ops, warping to a known hostile POS, cloaked, all sweet'n'dandy. BS not here, open up scanner, BS shows up within 1 AU. Quick, quick, quick, uncloak to launch a set of 3 AU's .... Whoops. Forgot the hostile POS. 
2. Roaming for kills in Pure Blind in a Ganka with a mate in an ECM Scorp. Spot another 'geddon, start to engage, scramble, web, zap zap he's into low shields already and ... target jammed? Of course my mate jammed the wrong Arma; victim escapes and must still be laughing.

He obviously failed his local check since he was asking in local "I need to go get my thorax and need an escort". Well the happy .5. gang obliged and sent him an invite, then proceeded to escort him in his shuttle to get his new shiny thorax only to pod him when he undocked in it :) During the entire escort he failed to realize that he was being escorted by the ENEMY fleet rather than his merry friends.
The TS was very funny after that incident =)
*edit: Not a big loss granted, but the way was damn funny ;)

Not the biggest in EVE, but I'll give a few of my blunders here:
Just yesterday, I was happily hunting and chaining a 3 BS spawn on one character and using the other to collect loot. As respawn was too slow for my liking, I was hitting all the belts in system as possible.
So, I kill a lesser spawn not worth chaining and warp my hauler in to pick up the loot. Get down to 2 cans left, and warp my Raven back to the chain belt to make more isk. I'm alt-tabbing between characters so I'm sure you know where this is going... I get distracted in teamspeak or corpchat or something, and after a few minutes realize I haven't checked on my hauler in a few minutes... Alt-tab to her at 20% armor with 3 BS's. Tried to warp out, but nope, didn't make it in time.
Now, the worst part of this is I had all of my main toon's back-up gear in a secure can as I had been moving it. Not much, but a few faction items and a handful of T2 stuff. That was the only can that didn't survive :(

Politically, Id say when CFS implemented their "anyone welcome" recruitment policy. We had every nubcake who wanted free bistot coming from everywhere. In the old days before MOO came everyone thought we were the bomb cause we had 80 member corps and ~2000 members, which was a huge number in the days when 5k was the max on the server on sunday.
Then moo came and out of 200 in alliance channels maybe 10 showed up to fight in blasterthrons with no MWD. Whee!
Personally, my biggest was flying AFK through PF-346.
The best one Ive witnessed was some drunk rusky who passed out and ended up flying AFK-autopilot through Querious during the height of the FIX Campaign to take FAT from Coalition. One of us blew him up and podkilled him with a crow. Think it was Sidon. +++

My biggest personal blunder was jumping into a .5 with a megathron and a -10 security status.
Also, on my first week of playing I got a shadow serpentis ion blaster, I though eh, serpentis suck, and refined it.

Two personal ones: During GNW, in Torrinos, loaded a haulerfull of named loot and whatnot to move and sell in Niyabainen and Yulai. Somewhere along the route I started backwards on the instas and realised I jumped in EC-P8R when Safespot and Tactical folders appeared in the right-click menu  My first officer I found, a 10 mil. something-or-other Blood. With 3 other BSes, 4 cruisers and 3 inties. Warped my Tempest in at 30 km and it melted in like 10 seconds. 
And a witnessed one: At the height of GNW, in Deklein, XZH I think, our glorious CEO and gang-leader at the moment, pulled a Blacklight 100 km off the PA-held station. Yelling and screaming on TS didn't work so we started guarding him. That worked ok for like 3-4 hours until we were just 6 BSes left, and 15 'nemies jumped in...

Back before they had alliance chat or secure channels for that matter alliances had to rely on basic chat channels. Some NORAD member invited a pirate into their official alliance chat. That pirate proceeded to invite his entire buddy list of pirates who proceeded to invite their entire buddy list. Well the situation exploded from there (with people logging on their noob alts and inviting everyone in noob chat) and the next thing you know the channel has hundreds of people in it from various factions most of whom have no clue whats going on. It was absolute madness that went on for about an hour and in the middle of it all Jade Constantine was RPing in it.

Biggest blunder? Probably not, but it was comedy gold.

From my own perspective and experience..
1. FA's mis-management of Period Basis and Delve, particularly them allowing their carebears to have the bigger voice and not realise the tactical significance of Delve. FA not going full bore to help FIX and therefore allowing BoB to get a foothold in Delve killed them... despite a lot of good guys in their alliance telling them to act and prevent it.
2. PA thinking RKK were not a threat. Hubris in extremis ended in the death of PA.
3. NORAD sticking to the "we are a defensive alliance" strategy and not learning to flex it's muscle or even take a seat at the table of inter-alliance politics until it was too late, they were culturaly unprepared for the assault the ex-CA corps brought upon them. Cost them a region they have never reclaimed.
4. The Merchant Marine's recruiting policy. Killed them from within and when their figurehead left Eve they could not recover. Could have been a rival for CLS.
5. CA allowing too many ego's at the top table with different agenda's, observation from outside only that one.
6. Me deciding playing Eve drunk was a good idea, has cost billions

I'm sorry Nick, but i gotta tell it.....
Back in the day when Nick Curso was one of my underlings, we had a mining op in providence that got crashed by pirates. Anyways he lost his ship was feeling a little bummed out and said he needed a bs to go join SA.
He only needed 1k of megacyte to build his ship, so i wired him the isk, was 18k PU in those days......
So he sets off afk in a hauler (his only ship) in secure empire space, to drop the megacyte off to get the ship built. Passes out ends up in a belt, and loses said hauler.
Nick is also famous for going npc'n without his hardners on so BoB guys if he says he's going npcin, everyone ask if his hardners are gone. (If I was given some isk everytime he would say, **** i just lost my mates ship someone gimmie some isk for a new one, rather then having to fork some out, id be richer then Khatred.)
Other one for us was setting up an alliance called Eve Player Alliance, at the same time the Eve Pirate Alliance was about, the guys that walked into our public channel were comedy.
Losing a geddon with no insurance 1 day before CCP instituted the 40% free insurance rule.
Setting auto pilot destination through low sec and not activating it and going on the phone to the gf. Lost a frigate to gate npc's, it had 1B of kaikka's mods in the hold. Managed to get another ship and was racing some random guy back to my can. Luckily only lost one item worth 15M isk. 99% of the above is said in RP. The Other 1% is personal, and should be taken that way. :P

I was hunting in low sec and half asleep and instaed to gate to run and catch a hostile, I jumped through (when aggroed) and then realized I jumped into the wrong system that just happened to be a .7. Good move I was in my scorp with a decent tank on it and only the amarr navy ships fired on me so I went 7.5 m/s back to the gate and tanked them the whole way and after about 20 minutes of a corp m8 bumping me in his hauler and me nossing him a bit, I made it back into the .4. Oh and btw, if I ever see that guy in the shuttle in the .7 that kept bumping me off course I'm going to tear you a new one. |

I liked the escort thing done by 5, still laughing.
About the raven, i found him couple jumps from fat, when iw as hunting a merlin. He was sitting 13km from the gate, so ijumped thrue and shot the merlin down. "was so happy the meriln jumped thrue". Toke my time picked up the loot and jumped back. The raven still sitts there.
I lock him,.. he dont lock back, not even auto lock back. I start orbit at 25km without mwd and start shooting missiles. And tell everybody els i got a raven at the gate, sadly they where occupied..
I keept shooting and when i was thrue the shild i was start hoping someting was messed up. Went in scrambled. Reloded and keept bashing. And pop he went, and then his pod. I go check the can-
Find megacyte and zyderine and alot off named l00t!. "he had prolly empty his station and was on his way out"... Was a fun thing =).

Ya, few people realize how cheaply all of the ceptor and dmg mod BPOs were selling in the old days. Lack of vision by the people who got lucky enough to get them off of their agents is probably the biggest collective blunder in eve history.
