The T2 kind, I've asked in 4 diffrent channels and have gotten 10 different answers, varying by a total of about 2 days.
I'll take straight answers or ones I have to do a little math for, as long as it's accurate

Sorry, I'm only up to T1 crystals at the moment.
They seem to last forever, but the T2 I have heard don't last nearly as long.

as far as I know its random, they could last 10 cycles or 100000 cycles I'm sure there's an averge time but its only an averge.

a while - it appears random. to give you an idea, i slotted in 3x omber II on my hulk.
#1 at 62%
#2 dead
#3 at 80%
i normally carry one refill =)

In my experience they have lasted anywhere from when you first use it until they are depleted. This varies depending on usage.

i had 3 on my covertor, lasted 15hours or so, "Tech II"... so pretty long i would say.

You can mouse over them while they are active. 0.?? is there somewhere, when they hit 1.00 they pop

Iv noticed 3-5 Hours on average, so around 4 hours depending on how many times you start and stop them. Each time a cycle completes the crystals takes 1-4% damage (That could be different, but its what I noticed) They also take damage if you turn off and on your lasers.
Hope this gives you a better idea :)
PS: Carry lots of refiles if your mining for more than 4 hours. I usually carry 2 refills of each type of rock im mining. Because I mine for anywhere between 2-12 hours at a time for the most part.
