lol, you're kidding right? See most of the Titan pilots in the game are actually good pilots who dont get themselves in that situation, or will undergo simple sequence of events.
a) Warp in (hopefully your dictors have fleet bubbled or they jumped into your gate camp of several deployed bubbles.)
b) Turn the F AROUND!!!!!!
c) make sure you know your warp out point hopefully a guy in line with an object you can align to.
DD) well you get it.
e) Warp the F out without cloaking!
f) once in safe spot cloak or warp to pos.
See most titan pilots of skill or brains can do this and thus not be in any real danger.

You missed a step. Correct order:

e) Warp the F out without cloaking!

g) die when you realize you never actually escaped, you just desynced

So did you DD before or after you realized you were desynched? According to your order you blew your load, then probably sat on the gate for some time before trying to warp and then died. Cause theoretically.... now I might be wrong, but if you blow your load, tacklers usually cant survive it!

So you warped to the gate close enough for tanked BS to scramble you? Not sure but I thought you still couldnt scramble a super cap? Either way warp into a spot about 400km off the gate. If you dont see any targets you are probably lagged, but if you are desynched choose warp out option. This means you warped right away so any dictor that has to travel 350km plus probably hasnt dropped a bubble on you yet. Half way up your warp meter blow your load, So any tacklers that are coming to you die, and so should most of the other ships, and oh yeah.... all dictor bubbles. You should be out and away. Now I may be completely worng since I dont encounter desynchs hardly at all, but this seems like even if everything goes completely wrong you will still survive it.
