For me it is the attributes.
The never ending thought of my attributes being set wrongly, but at the time I didnĘt really care, but now after 3years and max learning skills + 5implants and still looking over them thinking that if I had just set them different I would have had 5 more in per and 4 more in wis (I am a combat pilot).
And the pain it is to know that if I have to get the most out of it I will need to make a new char and lose those 25m+ sp I gathered.
That is what I find the harshest in eve.

The waiting for the skills I want, its also a good part of Eve - the anticipation of flying a new ship/ using a new weapon etc. ----

I would have to go with...not sure what the right word for it is, so I will call it the "No Luv" syndrome this game seems to foster.
It happens to all of us growing up in this game. For me, it was the usual "Concord Schmoncord, I hate cops!" "Besides they wont mind if I clear up a bit of this space junk to figure out how to shoot my guns" *squeaky scraping noise* "My ship turned into an egg!"
Anyways, the other day, there was this guy who had been in the republic military school for like 2 hours. The interesting thing was that he was blinky red to everybody in Rens! Not sure how he got that way, but I watched him get podded like 6 times while I was parked in the handicapped zone repping my hull.
He would drift out of the station in his reaper intermingled with a domni, a raven and a claymore and as soon as he hit the end of the launch rope *boom* 10 seconds later, here he comes again! 2. Losing in the hundreds of millions of isk from desync/lag.
3. Not being able to choose where 100% of your attributes go when starting a character.
4. Not being able to make a custom character that just gives you a "pool" of SP to spend on whatever skills you want when you start a new character.
5. Some of the skill choices CCP thought up when making a new character is absolutely pathetic. Caldari with Gunnery 5? Uh, yah.

I actually dont find it that harsh. Maybe im playing it wrong.  ---

The time it takes to skill up for a really good ship. However, this is also one of the best features because it means not everyone will be flying the same ships as me.
So overall, I like this harsh aspect!

What makes EVE harsh?
That this game can be as simple as pew-pew-pop or as frustratingly complex as trying to avoid having to buy something in Jita.
That you can spend hours fitting out your ship, studying numerous confusing tanking guides and curse the powers the be when 10 reasons is given why that you can't fit just that one more module on your ship... and to see it all go pop at the next gate-camp.
That you are so far behind most other players in skill-points that you can never quit.
That an high IQ may get you Mensa membership - but in EVE you can't even calculate the speed in which those 3 white bars turn into 3 red bars once the s*** starts to hit the fan.

the retrospective Eve manual, 20:20 hindsight is harsh.
Eve Manual section 42, subsection 4 "By now you will have realised that smartbombing that exploration plex was NOT a good idea, console yourself with the fact that the dev team is giggling its **** off, and waiting for your new battleship to be made so you can make this mistake elsewhere"
Carlsberg dont make Eve Pilots, but if they did, i wouldnt be one of them.

heh harbinger, 35millions + 10 mill insurance = 38.5 millions if go boom
full t2 fit..... many more millions, rigs more millions again (think, another ship + insurance)

Losses hurt. I can easily outfit a tackling int for 12-40mil depending on hull costs and fittings. 40mil being t2 setup without rigs. Or I could build a 200+mil monster int that could still pop to a bad luck or a lachesis.
Point is, even the 12mil fitting is a loss after the insurance.

Training to level 4 takes only a day or two on some kills and then training ot level 5 on that same skill will be like 20-30 something days? Yay I get to pay CCP to train only 1 skill a month to level 5 for $15 bucks.
