This happened to a fellow corporation member while she was mining in a high sec zone. I am the hauler for the corporation, and I was absent when this occurred. She just started playing EVE.
A mercenary/pirate attacked her while she waited for me to come haul her ore back to the corporation station. The strange thing was that CONCORD did not protect her. The mercenary scrambled her, and ransomed her, but she did not have enough ISK to pay, so she was destroyed.
The mercenary, pirate at this point, stole her ore, and sold it for 3 million ISK over the local channel. I felt guilty and decided justice was not served so I tried taking matters into my own hands. I tried to track him for a while, but could not find him since I had no leads to his current whereabouts. So I decided to set up a fake hit to drag him out. I assumed our corporation had kill rights on this guy, but I was wrong. With the help from another fellow corporation member we set up a fake hit on a fake target and lured the mercenary into a trap.
I ambushed him and tried to kill him. Caught up in the excitement, I ignored the "Dangerous Action" prompt and tried to do away with him when CONCORD came and destroyed me.
I know why I was killed, but I do not know why CONCORD did not aid my friend in the first place when the mercenary destroyed her ship and got away with her ore. Can someone explain to me how this could happen?

Only explanation I can see is the "pirate" jettisoned a can and swapped any Ores/loots in your Corpmates jetcan to his own.
Either your Corpmate shot the pirate (legally) because of this, or they "stole" it back thus flagging themselves to said pirate. To this end no kill rights are alotted as it was a perfectly legal tactic and attack.
Hopefully explained.
Shoot 1st, shoot some more, keep shooting, some more shot, and if anyones alive after ask a few questions. Just a soldier following orders.

Simply put - she did something which made herself a valid target for the pirate.
Whether that was stealing back from a can, automatically locking and accidentally activating a module on the pirate, etc.
Barring an exploit CONCORD don't randomly decide whether or not to come and punish in high sec, they either do or don't depending on whether the aggression is mutual or not. Unless your corp is at war with the pirate there is basically nothing that would prevent CONCORD intervening barring an action taken either consciously or subconsciously by your friend.

I had very similar with another character.
Pirate snuck up (i wasnt paying attention, my bad) and can flipped me When i 'stole' my goods back, i didnt get any warning about stealing from him tho.  As soon as i took my goods, i was toast.
Ive accepted the loss, and kudos'd said scoundrel, but i would like to know why i didnt get a warning. ive petitioned CCP to try and find out, but their official answer was ' We dont know'

Well, as explained before your mate did something to be flagged by the pirate. I had this the first time I got canflipped (had no more than a week playing the game) and got very upset because someone had just stolen 2 hours of hard mining from me... so I bravely opened fire on the red flag... popped a few seconds after... no signs of Concord. Lesson learned! And absolutely legal action on pirate's end because I shot him first... I lol at myself for such an incredible amount of noobiness!! shooting with a crappy single missile launcher from an Osprey to a well fitted Retribution!! 
Regarding that message not showing up, are you sure ya did not checked up the "don't show again" checkbox the first time it popped up?

For the ultimate in not jet-can mining for the new folks:
Sigil 1xMining Laser II 3xMedium Shield Extender 5xMining Laser Upgrade II
Should shrug off the rats as low as 0.6, Fill the hold in 35 minutes or so. And impervious to jet-can thieves. (Its about 1/3 as fast as mining with a maxed out racially appropriate frigate, but it might work out for you)

Comorant Highs: 125mm Railgun II x 3 75mm Railgun II x 4 E5 Vampire x 1
Mids: Med CL5 Shield Booster x 1 Small Brief Capacitor Overcharge x 1 X5 Prototype Engine Enervator x1 J5b prototype Warp Inhibitor x1
Low: Magnetic Vortex Stabalizer
Cargo: 170 of some type of ore 1000 rounds Caldari Navy Antimater 700 Rounds Spike S As many Cap 150s as I can fit
Note... make sure the flipper is in something smaller than a cruiser... Stealth bombers flipping FTW!!! Also works on Tech 1 frigates, waiting for an interceptor... idk about the assault frigates though... ē¦ |

I found that I had some how disabled the warnings. I got them re-enabled. There is a way to disable them but I don't. You won't see any warnings at all if it is disabled. Check your settings. Sorry I cant be of more help. Maybe the longtimers can tell you where to enable this.

Hit escape. Select "reset settings" tab. Find "confirm steal item" in the list. Click reset.

its gotta be something like that, tho i know ive never ticked the box as that toon only mines. Still as has been said before lesson learned. :) i'll go look at the settings tonight

This looks nice. Any particular reason you prefer the 200mm AC II over 150mm? But I guess with 3 X5s running tracking most certainly won't be the issue.

My corpie who's just a few weeks old got some nice kills with one last night, including a Jaguar and whatnot :P
