This is another instance of CCP's UI design being severely lacking in quality. The concept of feedback has gone entirely unnoticed to CCP's UI developers and this is the kind of result we get: people lose ships and the Internet tough guys come out in force to tell him he sucks at the game.
When I first started playing, I took a Thorax into a complex. It was a fairly easy complex to take a Thorax into, but I noobed it up and got myself swamped. I realised I was taking too much damage while I was halfway into armour, so I initiated a warp back to the entrance gate of the complex. Would have been fine were it not for the error message I received, which told me that the warp could not be initiated due to "proximity to natural phenomenon" that were blocking the warp. I knew you couldn't warp straight into a room of a complex, and that you couldn't MWD inside a complex, so I started looking around me for clouds, objects... anything that could be a phenomenon blocking warp. Eventually my ship exploded as I ran around this complex trying to find my way out of range of this phenomenon.
I later found out that the error message was simply doing a paltry job of telling me that I couldn't warp from inside a deadspace complex back to the entrance gate for that complex. I petitioned the loss on the grounds of the error message not telling me what I needed to know, and got my ship back (though it was insured, so I didn't end up much better off and removing the payout nearly sent me into negative balance).
Sometimes, deficiencies in the client lead to these kinds of losses, and it's in those times that CCP need to man up and accept that their UI caused the loss. The way the client displays information messages is a seriously awful part of the client. Had their developers paid attention in software engineering degree modules, the UI might not suck so much.
