Mine is the small/medium hybrid ammo icon mix up. Why on earth can't they swap those two icons? Medium ammo icon is clearly small and vice versa.
Compare projectile ammo icons:

(notice "small" icon cartridge can only hold 6 rounds while the "medium" icon can clearly hold at least 9 rounds...)
This simply will not do, until this serious gameplay affecting bug is fixed Eve is almost unplayable!

Maybe the box that the medium ammo is in is bigger and the picture was taken further away?

Missions that require you use an acceleration gate, only to find out the gate wont accept your ship.....after you have accepted the mission.

I think this was fixed (I couldn't save the can configuration before the latest patch). But if you mean when you assemble a new container, then yes, it is annoying.

Having to MANUALLY pause/resume training skills whenever you plug in implants or clonejump.
That, or having to wait 24 hours for a corp vote even if you are the CEO and/or only member and/or have 100% of corp shares in your wallet.
It's a tie between those

Can't get the overview to stay still.
How hard is it for, say my mission tab, for the overview to have gates at the top, rats just below, then cans and stuff after that? AND TO STAY THAT WAY>

Windows that don't stay where I put them.
Also, the combining of "Ships" and "Items" with any Can window, As far as my client is concerned they are the same thing. If you've experienced this you'll know what I mean.

say what you will about the ammo icons, the gyrostabiliser icons more than make up for it.
it's wonderful to see that CCP have embraced carbon-footprint reduction, and their ecological awareness is evident throughout Eve with the widespread recycling of popular sci-fi imagery and concepts.
keep up the good work CCP.

Definitely a peeve for me as well.
Here's another: having my jump animation interrupted when someone jumps through a gate at the same time I do. Makes me feel....Cheap.

unlocking bpo's. everytime they get moved its around 30 hours of endless clicking and god help you if you make a mistake. good thing they don't move very often.
close second would be boobs whining on the forums about something that should be something and why don't ccp something something with their somethings

being locked out of the overview for 'random time' when you jump through a gate, lose your ship.
That black screen on undock lasting more than 1 second and worse, the fact it needs to last that long
The tiny weeny session change timer (I'm getting old, my eyes!!)
Getting lock on a target and modules not activating
Any module lag makes me claw at my eyes
projectile weapons mandatory missing with last round (Seems to hit really well first shot, I assume to make up for it)
modules that stick on
modules that go off line when you jump clones
The use of economics on killboards to define efficiency (Artificial way to feel cool) .. hmm not really ccps to fix ;0

The lack of the promised "Eve-lite" to replace Classic - even with the graphics at the lowest setting Eve no longer runs effectively (on my work PC ).

That scanner probes don't stack automatically when they're recalled and placed in cargo. If you forget to stack them yourself and the launcher auto-reloads, you get only 1 probe in the launcher so you have to de-cloak and reload again. Most ulcer-inducing thing since Apocrypha at least.

Ships of the different races in the same class being so similar is my pet peeve, really. I'd love to see more distinctive differences between the races, giving them more character and individual unique tactics.
I know, there are already (some) differences but they are not significant enough for my tastes.
