Well, I just got creamed due to a stupid bug, first time it's happened to me.
I jump from Torrinos into EC-P8R, and when I get there, there are a bunch of DIE members camping the gate. I'm in my MWD fitted taranis, so I'm not too worried. I set my destination back to Torrinos, hit autopilot, and turn on my MWD.
Well, whaddya know. I zip up to the gate, and don't jump. "Stargate Jump in Progress." My ship dies. I get pod killed.
I've lost nearly a hundred million isk worth of implants due not to a war, or a pirate or any other legitimate means, but to a bug which has been in this game for all the months I've been playing.
Thanks CCP, I appreciate. How a bug like this can go on for so damned long without being fixed is simply beyond me. FIX IT NOW.
What's the most you've lost to a bug?
Don't include stupid stuff like your bad internet connection, that's not a bug.

Oh, and to top it off, the full insurance I had doesn't nearly cover my ship. Ridiculous.

Tempted to throw in a Starship Troopers reference here, but I know that'd be just mean. 
It's happened to me too -- sadly there's little that can be done. Sorry to hear about it happening to someone else.

That session change thingy has cost ppl more losses than you can possibly realise...
Since it`s there to prevent stuck chars etc due to the devs inability to come up with something else, I would imagine it`s only fair to think that you`ll get reimbursed... but you won`t

yup, i had this last night - lost a taranis and got podkilled, all after getting "your docking request has been accepted" sat there unable to do anything except watch my implants get fried.
it's annoying, but the GMs say they cant prove it - can't fault them really
they sent me a nice email saying they hoped i'd get back on my feet soon - which is exactly what i intend to do
anyway, i've wanted to lose them implants for months... now i have no fear

Yeah that's lame. Another bug that has presumably hit a few people is the cap bug with recently assembled ships. I had never experienced it until last weekend when my CEO asked me to move five newly built 'geddons. You undock showing full cap and full shields but in fact both are at 0.
I suppose in most situations you could hope to dock again before your ship blew but I wouldn't like to test that myself.
