Take it from a fellow researcher (although not in psychology): this form is bad, really, utterly bad.
as has been stated already: many of the questions don't fit to the possible answers. In some cases, giving a negative answer leaves it unclear which part of the question one disagrees with. But most importantly: the entire survey gives the impression that the questions are biased. Which (no matter whether you as the researcher are actually biased or not) makes it almost impossible to get "accurate/honest" answers, as everyone filling out this survey will instinctively "defend against the bias"

This survey reads more like a Scientology audit than a psych survey, it is total garbage. Don't do it.

Very good question, and definitely related to gaming! However you missed some important questions like:
- When I eat tomatoes, I often dress them up with mayonnaise. - Sometimes I feel like reading a magazine in the waiting room of my dentist - Have you ever peed while taking a shower? This test is hilarious.

I completed the survey - let the guy get his course credits.
Honestly though, these things seem so out of date now. I remember when roleplaying games and computer games singled you out as some sort of sad individual trying to escape their hum-drum existence in the "real-world"; but now gaming is so widespread and socially acceptable I think it would be hard to infer much from tests like this, other than what sort of people like taking tests.

Whoever designed this survey, I wish you would go through and move the listings on the answers around so they follow some standard.
Some of them are

If you are just trying to answer the questions without having to reread the same answers over and over, it gets annoying.

A while ago I took part in an extremely intensive psychology examination. The examination was spread out over three days, two hours per session, and was geared towards determining whether or not I had a learning disability (an IQ test was also included), since I never did any homework.
Irrelevant, but it determined I had none and I was just lazy, but here's why I mentioned it:
During the beginning portions of the test, where I filled out a questionnaire very similar to the one in the OP, I felt many of the same concerns you all have felt, while I was taking it. Many of the questions simply had no correct answer. For example "When you get high, do you feel like it's all you want to do?" which I couldn't answer, since I had never gotten high, and my only options were "Yes" or "No", both of which implied that I got high. Still others assumed I had some kind of personality quirk which I was fairly certain I did not, or seemed to be trying to make it look like I was an anger-prone, unstable psycho or something.
I complained to the assistant who was administering the test, and she said something about how "the questions were designed to look for things like that" or "it didn't matter" or something.
So I just answered them all as best as I could. I went back in to the office for the results about a month later, and they told me I'd tested "flat", that is, no personality problems at all.
These types of quizzes may *seem* like they are assuming something you would rather they not, but the truth is (if this guy knows what he's doing) they and their answers are arranged in such a fashion that the test-giver can glean actual, useful data from them. If you fill them out as honestly as you can, and you truly aren't like the person they seem to be describing in the questions, then the results will be that you are not.
Unless this guy's just a total noob and did a fail job at arranging the questions. Then he'll probably get a bunch of meaningless, unparsable data back. -
