Whats the stupidist thing you ever did in a pvp fight?
It could be a gate camp, a fleet battle, a small gang, or a solo fight
For me it was equipping a expensive tech 2 fitted thorax that I was going to use to kill a war target. This was a while back when a thorax had more drone space.
I equipped the thorax's high slots with Nosferatu's to power my tank and I figured the 8 heavy drones would do the work.
I undock and engage the target and when I tried to launch my drones they didnt launch, I sat there and died thinking that I was bugged.
It turns out when I read my killmail I realized I had put my 8 drones in my ships cargo hold and Not the drone bay

My dumbest?
I made it.... just now. Me and a few corp'ers were sitting SS'ed in Bhizheba, waiting for a few reinforcments to deal with a few Arcana Imperii LTD. After a beer and 4 mountain dews i had to go... badly. I mean, my teeth were floatin'. Did i log off? did i try to dock at a station without instadocks? NOPE!!!!! I ran to the b-room to relieve myself... and i come back to waking up in my new clone. Holy muther %%^^&&*&!!!!!!! I died without firing a shot . However... mad props to the Arcana crew. you are the first bunch of people to pod me since Zap. I mean, Lord Zap before he made his second character. So there!

You should get a 4L milk jug and keep it by your comp during battles. When you have to go, just stick your bird in there, cap it off and throw it off the deck. Rules of the road.

Well, virtually the one and only time I did PvP (rather than being ganked), I targetted someone in the fleet because their name sounded piratey and was promptly blown up by my allies drones...

Knowing there was a gatecamp on the far side, I jumped in solo anyway.
Then charged the sniping Tempests (200-ish km out) head-on in my Rifter.
I think I reduced the range by 1 km before I was in my pod... and another 50 meters before I said hello to my clone. 
Good times.

Turnes out npcs hated me more than him and he had t2 cruises and was fast enough to get me in his web....
Needless to say malediction went poof in an instant.

Firing Barrage s and foxfire rockets from my Jaguar at a Harpy, only to discover (after teh fight) that his base resists on the Damage I was doing were 60%+!
Of course he was even stupider and thought an effective armour tank was a SARII and a named WCS. That was when all that exp damage came good!

I saw a wartarget vagabond at a station, docked and equipped my raven for killing it (web drones, cruise, mwd, target painter, web, etc). Undocked, and when he engaged I slowed him down with the drones, got close, webbed him and started pounding on him.
He was dead meat, or so I though until he warped away. I had forgot to equip a scrambler.
You would think that over 1000 kills in EVE would have teached you to use a scrambler for 1-on-1 fights

I had set up a pair of tackler ships, a rifter and a stabber, because my corp had just been war decced on, we got word there was an enemy battleship in the area camping one of the stations we frequently use, so i co-ordinated with one of our battleships and we jumped right in...
...sadly i was in my stabber, as my rifter was in the station being camped, i jumped in and was immediately aquanted with the business end of several torpedoes. i managed to jump out, and he was also forced to jump away, allowing me to get my rifter.
now armed with my much smaller and faster frigate, the poor SOB couldn't hit me, and we managed to take him down (you know a battleship is in deep **** when the pilot ignores the batttleship hitting him to try using torpedoes on a rifter).
in the end we won the fight, and the pilot of the ship we downed later joined our corp ^_^

I don't know about the stupidest thing I ever did...but a few days ago, I was getting ready to run an L4 with my newest recruit. He undocks from the station first in his Caracal and tells me that there's a pirate right outside the station flashing red. I ask what ship the pirate's in and he says..."I don't know." Yes, he's a bit new. I made a guess that it was something relatively small and told him to lock it and warp scrambled it if the pirate attacked (have to wait until he attacks so that the pirate doesn't just dock, of course). I wanted him to get it locked down first because a lot of the pirates in the area are flying smaller ships than my Armageddon and they tend to run from situations that aren't vastly in their favor.
He targets the pirate, pirate attacks, he warp scrambles...
...my fresh new recruit sadly didn't recognize a Thanatos. He exploded as I was in the process of undocking. This is the last time I ever let him tell me he doesn't know what ship he's looking at.

With my other char i wanted to test my Lock time with the Stealth Bomber i had eventually got how i like. I was in Jita warped to a Planet where there where some shuttles sitting there, Pressed ALT-F1 for sensor booster. got that lock time.
Went to turn it off to see time and the ALT didnt register so i fired a cruise missile at the shuttle and got concorded

Was out hunting in a Crow with some Corpies in Rancer trying to lock down a lone gate camper... we got his SS and landed on top of him but he was already aligned and warped :(... However a previous target who we'd been hunting in another system (non flasher) had just warped to 100k off the gate and approx 60k from me. I kicked my MWD in and headed towards him. 'Follow me!' Goes into gang chat. 'Lark WTF?' Lock, scram, missiles away. Hmm he's doing a lot of damage for an Enyo... OH NOES SENTRIES WARP DAMN YOU!!! I died.
Also flying a Hawk in PvP...

first event was when I scrambled a ceptor in an empire war, watched him warp off, thought bugger, stabs, then got toasted by the guns as I had actually scrabled the station (that sucker wasnt going no-where!)
2nd idiotic thing was in my ceptor killing shippie, see a ranis in system, quickly slap on my gallente specific jammer, or as it turns out, what i thought was my gallente specific jammer  (it ended messilly, as you might expect )

Being bored enough to take on a Sleipner with two corpmates- me and one guy in an Ishkur, another guy in a Thorax (no T2 blasters).
Well.. I think if we had focused on the drones the Sleipner put out first we might have been okay, but there is NO way that 'rax was going to survive.
Also, not knowing what low security space was, when I was a n00b. 1 hour after getting my first cruiser, I lost it while ratting in Egghelende.

Being bait for a solo pirate/war target while I was in a small 5 man gang.
I was in a wolf, we knew the target was in a "cruiser class" ship according to intel. I figured I was going to die, so no biggie, but I would last long enough so gang could come in and get the kill.
Sounds simple enough right? RIGHT?!?!?!?!?!
Go out into the belts, 3rd belt, and here he comes in a recon ship... this is going to hurt, but hey I have my gang to back me up right!?!?!? I tell everyone to get ready in gang chat ( no ts at the time ). I lock him up he locks me up, I put out the XXX in gang chat to let my gang members know I am engaged and to warp to me now.
I am energy neuter'd, scrammed, and jammed. Oh joy! But I know my gang is coming.... 1 guy in my gang shows up and hooray! he is in a recon ship as well!
My ship goes pop like I expected, still only 1 guy in my gang there. Think to myself that more than one gang member really should have showed up. I warp my pod away. The target warps out.... the recon on my side didn't have a scram. Our BS backup got their just as the guy warped.
So, the BS beat the other 2 guys in gang....
When asked where my other gang mates were, the 1st guy that didn't show says, "Now I know what XXX means." The 2nd guy says, "I wasn't paying attention to gang chat, I was looking in local."
Moral - Be bait in a larger ship, that way you can survive the encounter and go kill the gang members that didn't back you up.

2nd day back in Eve after a quite few months back in EQ raiding (quit pre-RMR), first day out of the station and moving my Enyo out of Syndicate in to Empire until I get my feet back under me and knock the rust off.
Last 0.0 gate, I get jumped by a solo flycatcher...

Back when I was a noob, I was NPCing in lowsec for ISK to buy my first battleship. I was in a moa (lol), and while I was firing at a spawn, another moa warped in and started shooting them, too. I told him to leave my rats alone, but he wouldn't. I locked him and opened fire, and he did the same. When he warped off with heavy armor damage, I decided to go repair my own armor damage. I warped to the gate and got owned by the sentry guns.
Side note: A few weeks later, I was NPCing in the same system in my brand-new cruise-missile-toting raven, and the same guy warped into the belt with me, still in a moa. He ran away really fast and said, "Sorry, I'll leave your spawn alone." YARR
Later, when I was in Vertigo Coalition and living in Fountain, a couple buddies and I were bored after an aborted fleet op and wanted to shoot something. We saw a dot on the map in the northern reaches of the region, so we decided to head on up. When our four battleships got near the area, I started noticing the instas I was using were oddly named. They were partially in another language, but the gate names were clear enough, so I continued using them.
A few jumps later, the oddly-named bookmarks given me by an alliancemate became even more confusing, but I thought I'd figured out which bookmark was associated with which gate, so I gang-warped us to it.
Right as we hit warp, I noticed the full insta name underneath the "WARP DRIVE ACTIVE" text...and it ended in the letters "POS".
The insta name had been truncated on the right click menu, so the last part wasn't visible when I clicked "Warp to 15km". Only two of our four battleships made it out.
To this day, I still can't remember who gave me those instas (i.e. don't know who to pod).

I was trying out NPCing in .4 at three weeks old in my shiny (rusty) Caracal named Oberon. Suddenly, I was jumped by a Kestrel and another Caracal, and was promptly under attack. My response? Open fire on the Kessie with my Uber heavy missiles!
...At the end I was waking up in a new clone. I got the kestrel though
For the record, Im now on Oberon VII
To this day I wonder if I would have been better off firing at the Caracal given how long I lasted.

Reading this thread makes me glad I'm not the only one who's screwed up in PVP.
I was a week and a half old in a Kestrel that had only its highslots filled. We were in a war at the time, but that didn't mean a terrible lot to me. So I warp to a gate and I notice a rather unusual looking "Merlin" there... it's covered with dark / light blue guns and it's silver in color; it's also a war target... wtf?

Harassing miners in a Vexor, they take the bait. In warps Blahblahblah's Caracal. Simple enough fight, I move to engage... hm, this isn't going so well. Do my drone skills suck that badly? Down goes my armor, and soon I'm sitting in my pod with the caracal at 90% shields.
I finally look a little closer at the killmail, trying to figure out how I sucked so badly against such an "easy" target. Ohhh: Ship: Cerberus

Tried to warp from gate to gate when 3 hostiles were killing NPC's at gate, thought they'll keep focus on them. Got about insta-locked & WTFPWNscrambled.
Lost Badger II w/ 3 Local Hull Expands and Eagle in cargo blown up = 250m in my arse.
Wasn't a scrambler supposed to fail if the warp was already engaged ? or is it dictor bubbles only ?

Ive had a few in my time, first one being continually trying to fly through mara/passari back in the day :)
Most recent would be opening a IE window to look at some stuff and on closing it finding myself in the clone bay at a station in 0.0. AFK at a belt for the lose, lost my Brutix to a Harpy and some belt rats :(

my worst blunder was attempting to get to the station to kill something, and clicking on the station in space. Little did I know, the station is at the same moon as a "death star" pos, and pretty much 95% of the time, you click the moon rather than the station to warp to. Bye-bye super expensive-but-useless-when-webbed-and-scrammed Vagabond...

Aligning in the opposite direction of a tristan's motion on a gatecamp in a ferox, and causing enough transversal that my guns wouldnt hit and heavies werent packing a punch, so he escaped and i was just getting armor bleed (3 months old, hadn't a clue what a hardener was), i hit the safe spot bookmark, which turned out to be the other gate. Escaped that one in very low structure.
Not pvp, but a few weeks later a gang of mine chased some people all the way to Harroule, and decided to rat for a day (in thhe same ferox). I fell in love with 0.0 then, and started ratting (cause outlaw was getting annoying). Didn't know i needed to put asteroid belt battleships on overview, and never even saw them sitting there... Pummeling...

So I'm travelling through several low sec systems in my Rifter looking for targets. After several dull/empty systems I stumble upon one system with only one other person in local. I quickly narrow down what belt they are mining in. Warp in, AB to target, scramble, web, open fire. The ship is going down very quickly, I'm almost to structure when I lose the target. I think to myself "Why do I not have the target locked anymore? Oh, they must have jammed me!" I can see all these flashing yellow squares and wonder where all these guys came from, since there are still only 2 people in the system. It's then I realize that it's CONCORD that's jamming me, and I see that I am in a .5 sec system.... Whoops The one high-sec system I had to travel through is the one where I decide to aggress. Nothing left to do but wait for the next 2 volleys to finish me off...
So I guess the stupidest thing I've done in PvP so far (and it's been a short career) is actually initiate combat.

A mate warps in and we go to set up a gang.
I hit YES to accept the gang attempt, and suddenly I've got a trio of interceptors, two ravens, and a bunch of HACs blowing the hell out of my BC.
Someone else had offered to gang with me, at exactly the wrong moment, and I hadn't read the pop-up properly before accepting it.

funny thing is, a mate in my corp engaged him 10 mnutes later in a claw and lost... I was like, i'm in my ranis, i'm hard and i'll kill him before he even gets a chance to lock....

was a few months old, and was feeling confident in my osprey for NPCing, ruppy warps in, locks, and i warp away to the gate, ruppy warps to me again, and locks, i lock back, thinking WTF is going on.....hands shaking, I think if i get the first hit in i may have a chance.....

Found a target with the scanner, landed at the belt and he was there, got him locked and scrambled.
He tries to run out of scrambler range but I'm not worried cos I'm faster than him. I should be faster than him... hey I'm slow, very slow, wtf ?? He warps out.
I forgot I had t2 ammo loaded, the ones that give speed penalty..

Camping with sniper mates around bubbles in a close range Domi fitted with a cloak. We got word of a large fleet moving to our camp (too large to handle) everyone cloaks. I try to warp away to a SS, and forget to cloak myself. Ergo : slow ass turning Domi 4tl and 5 seconds later 100M+ ISK lost and podded.

Guy steals from a corp members can - gets aggro. I follow him in a .4 in my gisti-crow. Engaged his destroyer on the other gate and kill him. Instinctivly go for and kill the pod. Gate sentries proceed to engage me. I was shocked at first (forgot I can't pod him) and hestitated to warp out (I came to a stop after the kill). Needless to say, lost the crow, along with my Gistii-mwd and faction scrambler. Very costly loss for just a destroyer kill. Live 'n learn.

Watched 5 on 5 tournament. Was doing vengance mission in apoc with all modulated tachs, all tech II low and med as well. the bob match came on, you know the one i mean, the best match ever. well watched a little too long and lost ship.
lesson learned: while watching 5 on 5 on EVEtv stay docked at pos or station till match is over

honestly - 9 time out of 10 that you die, you pretty much immediately realize that you should have done something different. sometimes you make same mistake a lot ()
main thing weirda do:
-getting agro timer, when you are better off jumping and engaging less (ie. not all) targets
-forgetting to turn off mwd and drain own cap while orbitting slow and taking full damage from missiles in interceptor
-screwing up module activation by preactivation outside of range (they keep blinky after target aquired)
-bubbling self and pod with own interdictor bubble (added bonus when you get you gangmates too!)
-too many other embarassing thing

1. while pirating i found a mining barge at a belt, warped in 30km away from him and he doesnt seem to be moving....weird. I engage and soon after promptly get killed by CONCORD!! >_> Yes i was in a 0.5 system
2. geddon coming up on scan while i warp to the station so i can fit up my new vexor. I warp to belt and engage...where the hell are my friggin drones?! oh right they are in the station..
