I had a pure research alt character. She had no faction standing with any entity and my corpmates were happy about that since it doesn't get averaged into corporate faction composition (which was high) and they wanted the option to put up a high sec POS in the future.
After reading on the forums that it was safe to ignore or refuse storyline missions, I started missioning with this research alt to increase it's personal standing with one of the npc research corps. I wanted just the corp standing, but absolutely no faction standing whatsoever. This was going just fine until I started getting storyline missions -which I ignored.
But then I noticed that my character was now included in my corp's faction standings average and was now pulling it down dramatically. Further investigation by viewing my char's faction, right clicking and viewing transactions, showed a different story.
These ignored storyline missions were logging a very tiny -0.015 % faction loss against my character. Viewed in the character sheet this value is round down to -0.0 so you can't see it. Even one is enough to ruin my neutral standing, but I had several. My conclusion is that each storyline agent can only give out one important mission to you at a time and they are good for approx. 1 week. If you do enough missions to get another storyline mission from the same agent, it seems to quietly auto-fail the last important mission and replace it with a new one -each one give you this tiny faction hit.
For most ppl, they may still decide it's worth ignoring these missions. I've not yet tested flying all the way to the agent and directly refusing them but I'm guessing it's the same.
So if you're in a small corp working on high corp standing beware. The only way to keep an alt char purely neutral is to never have it get a storyline mission.

Yes, but to decline a storyline mission under any circumstances will result in a standing hit, they are different from normal missions.
So basicaly, what the OP said is true. If you want neutral faction than you can never get a storyline even offered to you.

If this is true it's changed in the last year. I used to run level 4 courier missions when I played on a craptop (it could *barely* deal with a level 1 shooty only if I zoomed totally in and looked straight down on my ship). If my storyline agent gave me something non-courier, I rejected it. I never suffered standings hits then.
That said, the quickest way (but somewhat boring) to increase faction standings, and possibly corp standings, is to run courier agents with same corp courier storyline agents. On a good set of missions you could turn over storylines every 2 hours. In those days you also got offered implants every storyline (by level) and that was the main source.

Wrong, declining an offered but not yet accepted storyline will have no standings impact.

To sum it up:
1) after getting a storyline mission from a agent you have a week to 2) accept or 3) decline it or it will 4) expire;
2-a) if you accept the mission and you complete it you get a gain in standing with some faction and a loss with other factions
2-b) if you fail the mission you lose standing with some faction (in the past you gained standing with the opposite factions but it it was removed as far as I know)
2-c)if you refuse the mission after accepting it you lose standing with some faction
3) decline it without accepting it. No standing gain or loss.
4) if you let it expire you lose standing with some faction
Additional note: a) you can have several storyline open with different storyline agents, they don't cancel each other. b) storylines give a large boost to your corporation standing for the agent corporation, so if you are after R&D agents check how many mission you have done and try to get the 15th in the same station of a storyline agents of the corporation with wich you are trying to raise your standing.

I let storylines expire all the time and I've never had a standings hit for it. --

I think there is a difference that should be noted.
Talking to the agent and saying 'have you any work for me' but ignoring what he offers (not accepting the mission) is different from you getting a message on evemail saying 'please report to me for a mission'.
In the latter instance, the offer is 'there' but you have not been offered it in person and so ignoring it will not take a standing hit.
In the former, you have been 'formally offered' it and so must complete it to avoid the standing hit for failure.

You can say NO to agent every four hours without standings hit.

I've never had a faction drop from a declined or ignored / time expired storyline mission. Or is this some new thing since the last patch?

I ignored a storyline mission offer that would've involved travelling 16 jumps out of my way to decline in person and didn't receive any standing penalty.

Like that would happen. Courier missions are the way to get lots of storylines fast. When I'm cooking I can almost get one level 4 storyline per hour.

I can confirm this at least for regular missions. If you let the mission offer expire without accepting / declining it, you will get standing loss with the corporation. --------------- Hey, where's my portrait |

This is false. If you run enough regular missions to trigger another storyline mission, it will find the next closest storyline agent for your next storyline.
I've personally had seven storylines piled up before acting on them. I had found a location with one storyline agent in system, and six storyline agents 3 jumps out in a circle. I found it convenient to pile them up as I got offered the 'Preparation for War' storyline a lot, so I'd just stock up on kernite and just make a circle. ---
