I like antimatter for short and lead for medium/long range, the lead gives no range penalty but saves on cap, and still does decent damage up to my max lock range.

Well lets see...using 250mm Medium rails with iron charges at 48km and I hit for about 20-30 per hit, when I hit at all...which is about 50% of the time, so I don't think it's quite as bad as you make it out to be, you're just over-reacting as usu...

erm...first of all what are you flying and second of all what ammo are you using. Also are you between the turret's optimal range and the falloff range? if not that explains your problem.

I have had this happen as well but I don't know how to reproduce it. After the MWD bug happens I am unable to warp very far until I dock or restart the client.
