Here's me in my Firetail roaming around lowsec when a corpy reports a Tempest in a belt. He has already engaged and I join in as the Thorax gets popped. I continue the assault, knowing I have nothing to fear from a BS, start taking out drones and wait for reinforcements or him to slowly fail as I have enough ammo.
Suddenly Nyx.
I'm all like "lolololo cyno***, can't touch this". He's all like *neut, web, boom*. I cry.

I lost a 2b fitted Tengu to concord once...was engaging pirates in low sec then warped to a gate and clicked "jump" I got the agro warning and instead of clicking "No, I don't want to get wtfpwned by Concord" I wrote a message in local and hit "enter"....turns out that an open dialog in eve will hijack the enter key even if its not currently selected/in front 
Once arrived on the other side (highsec) I sat there cloaked for a while being like "WTF do I do now..." until I tried to quickly align and get out but yeah that just doesn't work.
...so stupid

Lost a half a bil loki because I forgot to switch the interdiction nullifier for something more suited to lowsec gate piracy.
Died to gate guns and a rupture. *sigh*

I was hauling today and forgot to take my faction and deadspace mods in my usual SB for the run.
About 500m loss, I have a 500m wallet as well.

Well, let's see...losing not one, but two Machariels to faction police, due to alt-tabbing on autopilot...
Then a prowler got killed by police because I didn't want to hand over my recreational drugs.
I am also probably one of the few people who managed to get killed by a pleasure hub.

I was in my home 0.0 system and in a covert ops frig on my way to high sec carrying high value items to sell. I had to go afk and didn't pay attention to where I was. Turns out I was at zero on a gate not cloaked and afk . I was soon blown up and podded by a wandering neut.

My most embarasing loss was a few years ago, when I was flipping cans in heimatar.
I had my main jetting cans in a dominix while flipping their cans with my alt in her mammoth.
I had it all figured out... if they wanted their ore back, they had to go through the Dominix and I was chestbeating in local about it.
Suddenly someone grew a pair and popped the alt mammoth in front of the Dominix while I was smacking in local.
I made a swift decission that they wouldn't get any ore back from the mammoth wreck and popped it with my main.
Problem was that I failed to remember my alt was in a noob corp and I got concordokkend in front of the guys I was smacking.

My most embarrassing loss. Running a C4 sleeper site with my corp in an Abbadon,which was part of an rr gang.Got complacent,as we were moving through the sites well and took my eye of d/scan. Got jumped by a roaming gang,was primaried by the opposition pointed etc,but was shocked how quick my Abby went down!,warped my pod away and looked at the kill mail. Infact two of our rr gang had panicked and instead of training there weapons on the enemy were shooting at me
One corp member even got the final blow!
As you can imagine i wasent a happy bunny,my old corp went the way of the dodo, but my new corp im in, has the same two gunho players! (good mates,who i was still remind when we venture into wh space..please check targets first!)

I flew my Arazu cloaked a few kilometers away from a station where a Zealot was harassing my corp mates at range. They were talking about how we would kill if we got it pointed and so forth. I decided to play hero and de-cloaked my Arazu and pointed the Zealot but he reacted too quick and soon enough my Arazu was down.

When wormhole space was first introduced, me a couple of corp mates moved into a class 3 wormhole. We spent about 6 months in there, all the isk we made from the sleepers (which was a lot) we spent on shiny toys (faction battleships, T2 battleships, tons of faction modules, loads of T2 cruisers, etc), which we took into the wormhole. We also built 2 carriers in the system.
Well, one night I did the POS fueling while drunk and didn't load enough robotics.
A couple of days later I logged on to find the POS had gone, the fuel had run out and the shield had dropped. The whole place had been cleaned out.
Annoyingly the place was cleaned out 2 hours after the fuel ran out, and that was only the third time we had seen other people in our system for the whole 6 months.
Total loss was about 20 billion isk

This happened with a different character years ago. I was a noob and just saved up and got my fully T1 fitted Megathron. At the time I felt like I had the best ship in the game. If you guys remember when you are new and flying frigates and cruisers for a month or two while bunch of people are flying around in battleships you feel, you know small... anyhow, as I was feeling completely invincible in my Megathron, a seasoned corpmate asked if anyone wanted to join their gang to do a lvl4 mission. I offered assistance, but they were more interested in getting corp mates running a mission together. Basically nobody needed my help but i went anyways, got in fleet, warped to them, went thru the acceleration gate, told the other 2 corp mates in fleet not worry that i will tank them all in my megathron (my exact words)and died horribly about 2 minutes after...
It really does not get any more embarrassing than that.
Edit: Pvp loss would be a 550mil Iteron markIII loss to a gate camp in catch. What makes it embarrassing is that i wasnt looking at the intel channels, i was not using a scout al lthough it was avalible to me as my own alt, and was offered by alliance mates... I also had cloak that I didint think of using because i was too busy cussing at my luck...

Way back when I was a nub in a Punisher (back in Ye Olde January 2010), I was in a small gang in Amarr lowsec, near Providence. We were chasing some reds around and managed to catch a one on a gate. I went for the tackle, got the Concord notice, dismissed it, tackled the red. Red died about 30 seconds later.
Noticed some of fleet mates were being shot by these bright yellow beams from near the gate. I figured, "Huh, well, they're not shooting me, so I guess I'm okay." Puttered over to the wreck with the intention of looting it. Ten seconds later, there's a bright yellow flash and my shields are gone and my armor's melting. I have just enough time to go *erp* before there's a second flash and I'm sitting in a pod.
And that's how I learned how gate guns work.

I also lost a tackling incursus to a corpie. we were waiting somewhere, he had his drones out and I started getting bored, so I tackled his curse. The fleet of warriors did not agree.
Another time we were camping at a station, and I again was tackler (maulus with res boost) a corpie of mine targetted me for remote repping, but started shooting right away when the target came out. I notice, flee, and the target does likewise. I had about 40% hull left when the shooting stopped

i think one of the first kills i had in low-sec after i killed him and i forgot i had the criminal timer on me, so i warped to the gate and gate guns blew me up.
Another time i thought the war was over so i auto-piloted my iteron V with cargo. When i got back i was in station with a new clone, lost alot of implants too haha.

I was in our wormhole POS and it was currently being shot at by a Moros. He was casually taking out the guns one at a time.He'd fire with his turrets at one gun and set his drones on another. I was the only one online in my corp at this point so I had no clue what to do.
I decided that since there was no way in hell I could take the Moros down I would attempt to kill his drones instead so it would cost him at least a little more cash than it would in ammo anyway. (Orge II's so it would've cost him a few mil) I warp out to a planet, cloak on the way, then warp back to outside our POS, line up on his drones that were orbitting one of our guns and with a celestial behind it, uncloak, drop bomb, warp to celestial. Unfortunately, and I should have seen this coming, it did nothing at all.
So I warp back into the POS and just watch as more of our guns fall to his. Then he starts getting refueled by a Prowler... Now that's something I can shoot! So I do the same warping business and try to catch the prowler with a few bombs to no avail. He was too fast at aligning to get with bombs. So I thought I'd be cheeky. I set orbit around the Moros (cloaked, obviously) waiting for the prowler to uncloak to give more Stront to the dread. He did, I uncloaked, pointed, and took it out right under the dread's nose.

I was well chuffed with the kill. I smacked a bit in local, as you do, and felt good about myself. I got myself back in our POS and clicked toward the corp hangar to reload on bombs and torps incase he bought another and tried the same thing. I also get a mail from a dude not in our Alliance asking what was going on. So I type away. I'm half way through the mail-
BOOM.
I'm in my pod...
I had manually clicked toward the corp hangar by habit and not realised. I'd been too engrossed in the mail to see that I had flown right passed the hangar and out the other side of the force field. I congratulated the dread pilot on his lucky kill because of my stupidity. He, however, informed me that he had done nothing and was laughing incontrollably...
I checked the killmail and almost fell off my chair in hysterics. My own POS had killed me due to 'attack on aggression' being turned on and my bombs on his drones hitting our guns too.
Ridiculous KM
This killmail still sends me in to fits of giggles.

Someone got blown up in a stealth bomber when i azzploded the station in damsel in distress.
He was gonna try to steal mission item not knowing station did 3K explosive damage to 25km radius.

Carebear story-I am mining in high sec like I always do, when in local chat I see a warning of a can flipper. I set him to "terrible" standing so his ship will stand out if he comes to my happy mining spot to cause trouble. At that point I start to remember a YouTube video I saw where a Hulk kills an un-suspecting would-be ganker by fitting his Hulk with a point, web, etc.
This can flipper is only days old and is flying an Iteron mark I. So I decide I'm going to teach him a lesson and be the hero of the day. So, being the PVP prodigy that i am, I fit my Hulk with a T1 web and scram and some T1 Hobgoblins (remember, I'm a pvp god), and set out to bait this menace and make short work of him.
First I get a cargo full of Plagioclase and jetcan. Bait set. He's as good as dead. It takes a while, but Mr. Can Flipper shows up in my belt and I see the distance closing. His tactic is to align to the station, and just before he hits warp, he takes the loot. He did exactly that and I wasn't able to lock him in time...hard earned ore gone! But lesson learned.
So I deploy another trap and he comes back to try his luck again, after the 15 minutes aggro timer is up. This time I'm ready. I'm literally shaking from the anticipation of what will undoubtedly be my first pvp kill, in a Hulk none-the-less! So he starts closing the distance again, and I target his Itty and deploy my drones. As soon as he gets within 2500m of my can I hit him with scram and web and a flock of Hob 1's. In hind sight I prolly should've read that warning pop-up a little better, as my Hulk was CONCORDED back into the space dust from which it was created. If I would've read the pop-up, he surely would've gotten away (which he did anyway). So that's how I learned how to lose a Hulk in less than 5 seconds to an Itty Mark I.
The can flipper was nice enough to let everyone else in system know of my stupidity simply by asking in local "someone lose a Hulk?". F***er!!!

My best loss yet (and I'm about 3 months old  ) is when I was mining about a week ago. A ship warped in at 50km, in my curiosity I looked at it and saw a Bellicose hull going at about 1km/s. I warped out, got my mammoth and returned to pick up my can, which he had flipped. Me,seeing no-one there did a little sly re-steal, and lo-and behold, he uncloaks, and murders my mammoth. Should have taken an ECM

I lost a Hawk to a dual-Jaguar team. The embarrassing part isn't the fight, or that I lost the ship, it is the fact that I undocked in a Hawk to begin with.
