3 times. No, wait, 4. I went AFK in a shuttle once during a war.
I've self-destructed my pods too many times to count though. I'm lazy, never have implants and my clones aren't expensive so I almost always SD pods if I'm in 0.0. _________
In war there is only relentless and pointless quotes about war such as this one |

First time courtesy of Anister of Black Omega. I was two days into Millennium and at empire war with them. I fell for the frig trap and got subsequetly popped in my thorax. I was too dazzled to warp the pod since it must have been like my 3rd PvP experience ever. I got my revenge later tho =).
Second time I got smartbombed by a Typhoon in my Taranis because I went afk while watching over a station thinkin I was offgrid. Except that the covert was within 100km.
Third time was because a gang member requested help, so I warped my brave Ishkur to him only to be greeted by 6 inties. My ship got stuck in the moon (was in EO2-IK, I'm sure many people know what particular moon I mean) and I was in a cloning station before I saw my ship explode. My can was still stuck inside that moon when I returned there later that evening :-)
Oh and now that I'm thinking, I got smartbombed in a shuttle by friendly fire once, because the guy was really getting off on getting killmails and I happened to undock at the same time as an enemy Stilletto. If course said Stilletto was in a warp bubble and there were like 30 friendlies on that station, but nonetheless he hit the bombs and I ended up in Nonni. Oh and he blamed me for undocking too 
So that'd be four times.

once, I was poking around in 0.0 and was chased by a small AF group from NBSI for about 8 jumps. I wasn't watching the map, and was run straight into a big chokepoint camp.

If I'm in a fight and I don't feel like flying back, there's no resaon to run, as clones are dirt cheap, and I don't use implants...

Podded 3 times on my regular (secret) character. Once during war, and twice while jumping a couple of systems while going to the kitchen irl to get some coffee. I figured: What are the odds of that happening twice in only a few days? 
I didn't do that again.

Once on this account. My fault for being in the wrong place at the wrong time but lag stopped my autopilot warping my pod out. Was back in January I think.
Twice on another account. Once I got lagged or perhaps 'detained' after I accidently accepted a convo invite while heading toward an insta to re-enter Empire.
Second time the char was hauling and first the a/b got lagged then yet again lag prevented the pod from warping out (I was actually using autopilot to approach the gate while trying to fend off the ganker).
I never have got into PvP and tbh the high likelyhood of losing my pod does nothing to encourage me. Luckily there's enough non-PvP content in Eve to keep me happy and I've always preferred building to destroying  -- (Battle hardened miner)

Twice by hostile pilots. One of two corpses sits in a corpmate's hangar.
Been clones like 15 times or more, though. Either clonejumps or safespotting -> self-destruct so that enemy doesn't get corpse.
Suffice to say that I don't wear implants. Ever ---

Once in a Battle , but im not happy though my clone has no belly button and im now built like a action man in the pants department :( ive complained to the clone makkers but no refunds given ive been told , and i dont want to get podded again as ive no idea what else will be missing on my next clone :(

once, by curse... who said they wouldn't pod me... one guy wasn't paying attention to his corp chat and podded me anyway when I jumped to gate... got my implants and clone refunded by them though

Podded several times
Clone jumped even more times.
And even lost SP once, because i was clone jumping, and was in such a hurry to get to a fight, that i forgot to get a clone and then clone jumped again, to get closer to the fight Lost like 15 days of a lvl 5 skill 
And btw no one messes with my brain, so no implants for me

Missile blast radius used to be in-game. I vaguely remember Concord using missiles as well back then. ---

Used to get podded loads of times when i started playing. Joining CA during the height of the CA/SE war when you have only been playing 3 weeks brings hard lessons :D
I got podded a couple of weeks ago for the first time in months. by a MUNINN, of all bloody ships a muninn gets a lock on my pod and hits me with a howitzer shell thats probably larger than my pod. Damn ship lag.

4 times in my life but 2 times in the last days.
3 days ago I was interested in jamming and built a blackbird with all nasty stuff to tease lonely frigs: Three jammers, scrambler, webber, med nos and assault launchers, but only a repper as little defense. Then I travelled alone through 0.0 waiting for an assault frig to attack my weak looking ship. Two intis attacked me and I thought: "Yes !!!". Both got jammed, the claw was totally trapped, had no chance to escape and died horrible slow death. Guess it took minutes, the assault launchers were weaker than I thought, then the other inti escaped.
They came back with a mate in a Munnin. Game over for me. So finally they won and I got pod killed. But great fun anyway.

First time it reallyhurted. I was still flying cruiser due to lack of isk but I had some uber implants fitted 
(on the plus side, I managed to take one of the ebil ganksquad assault frigs with me )

hehe, a little while ago i upset my corp and, unfortunately for me, the alliance they were attached to. as a leaving present, they pod killed me 3 times in 10 minutes... nearly qualified for a loyalty reward with the cloning company.
i remember corp chat well that day: "is it possible to stack corpses?"

Never by a hostile pilot.. which still makes me wonder how thats possible if you pvp pretty much every day. 
I think karma is waiting for me to finish my set first.

We need a 'times podded' counter for our characters.
I don't want to think of how many times I've sucked vacuum. Surprisingly few of them weren't politically (corp/alliance) motivated.

I'm intrigued as to how you have two slot 6's.
But still, posting pics like that on the forum, I doubt you'll ever get podded. It would make much more sense to ransom you if someone ever gets a lock on your pod :-/
Edit: As for me, the only time I remember off hand was when my ship got blown up in a fleet fight, and I thought I'd warp to a stargate so I could instajump out through the other stargate, but I warped in at 20km by mistake and the whole enemy fleet was there. A Vagabond locked me before I'd loaded :<, geez.
But still, I deserved it for being a tard. Oh yeah, and there was the 4c- lagfest, but that was reimbursed.
I'm certain before I got onto PvP I got podded a few times. I have vague memories of travelling afk through 0.0 and then not being in 0.0 anymore when I got back to the computer.

Haven't been podded yet, never had lag, always got away instantly without ever logging off. I was attacked by one smartbomb though the other day, barely survived ^_^

I've been into pvp for over two years, ever since my character was created. I used to get poded a lot, but nowadays the only times I die is when I run two clients at once and lag, or pod jump (or lousy PA BM's). In total I estimate around 15 times. ---------- Fedaykin
