I just recently lost my tengu in nullsec. Due to my own stubbornness and stupidity. I deserved the lost 100%.

None of mine are particularly impressive. I've managed to do a pretty good job of hanging onto my more expensive ships. Guess I'll just post a few of the dumb losses I recall.
Loss: Raven (~2004) Reason: Not paying attention Aggressor: CONCORD. Some guy popped my buddy in the low sec system next door, I jumped in guns blazing, chased him around, he jumped out to high sec, I followed. Learned a valuable lesson about aggression timers.
Loss: Cerberus (~2004) Reason: AFK due to a horny girlfriend Aggressor: A static planet spawn (these dont exist anymore) of 4 or 5 Guristas Terrorist cruisers. The sex was good though.
Loss: Raptor filled with ~300m worth of faction gear (2005) Reason: Used the wrong set of insta-bookmarks (before Warp to 0). Aggressor: A fleet of gate campers in HED-GP. I should have been paying attention to where I was going.
Loss: Eagle (2006 or 2007) Reason: Poor forethought + bad luck. Aggressor: Death star POS in EC-P8R. My alliance had locked down EC-P8R and destroyed all but 1 hostile POS in system. We were camping the gate with about 100 ships. I was a sniper Eagle, decided I could get better shots from another vantage point, warped to a random moon out of dozens in-system... it happened to be where the sole POS was sitting.
Loss: Scorpion (2008) Reason: Overconfident Aggressor: Some random war-dec corp. I tried to jam a few targets simultaneously, missed some cycles. I'd had this ship since 2004, had used it in a few high-sec wars and never lost it.
Loss: Ishtar (April 2011) Reason: AFK, phone call, ganked. Aggressor: Veteran pilot from 2003 in a Dramiel. Was passive tanking a Sanctum out in null, got a call that my fiancee's grandfather was in the hospital dying, forgot about EVE. 30 minutes later I end the call and get back to my PC to find a Dramiel had just managed cut through my armor. By the time I got drones out, I was done. Nice fellow though. He went and killed a Tengu after I paid him the ransom for my pod.

Hulk with full T2 fittings and arkonor crystals. Due to wife demanding my attention over a pointless issue that could have waited, while a Hawk and Retribution snuck up on d-scan and slammed me. About 150 mill, I guess, so not too bad.

Category 1: Laziness/noobiness
For some reason I decided to haul 820k units of Zydrine in a Badger II. Lost it in Jita to a suicide gank, 780k of it dropped.
My second highest loss would be my full +5 implant set lost while transiting Rancer in a shuttle. Facepalm when I realized what system I was in - I was just autopiloting on a new install that didn't have Rancer on avoid. |

Lost a zealot due to a surprising result
Was chasing a war target navy thron around with my gang (about 8-9 of us, mostly hacs/recons).
Caught the navy thron and we were mwd'ing around it and slowly killing it on the gate after chasing it a few jumps. A neutral dominix appears and starts gunning for me, I'm thinking "lol what". All of a sudden even though I'm 30k away from both I'm heavily webbed and before I know it the Domi + Navythron are at zero on me beating the **** out of me, I try and get back to the gate with help from a friendly Scimi but alas I pop.
Turns out there was a single low sec between the 13 jumps we were traveling and who knew people actually use heavy webbing drones! 
Domi died and Navy thron was ganked later in the week so :revenge:
Not a very expensive loss but I never buy expensive things

My main got lazy and lost a Vargur on a mish. Confused it with another one, shot the wrong trigger, got blobbed--you know the story. Warped off my pod in disgust and failed the mission... Then I realized I hadn't bookmarked the wreck! And of course all three faction gyros dropped... I switched to exploration for a while after that. All told, probably 800-900M lost (180M of which I could have gotten back if I'd BM'd before failing the mission...).

Billion ISK in rigged ships stolen by a crooked freighter pilot from my previous corp. Corp was relocating to another region and :welp: The loss constituted about 90% of my assets, ISK-wise. Made the most of it though. It was kind of fun starting over with a nearly empty hangar and barely 100 mil. I developed a more streamlined perspective on asset ownership.

I also got ganked in Delve flying a faction fitted Vargur. I got a phone call from my GF and got distracted. I looked back to see an Arazu tackling me. An alliance mate came to help with a Blackbird, but warped out because he was afraid of losing his ship! I tanked them for a while before a Curse showed up. --- This doesn't even seem to be a regular case of rats fleeing the sinking ship. Here is one I posted before. Not a loss for me (it was close call) but for my friend instead.
I have a friend who, despite his 'enthusiasm' for internet spaceships, is a bit on the 'green' side (bordering on 'dangerously fail') when it comes to logistics and 'strategy.'
Over the course of a few weeks he complained about how no one in his corp wanted to PvP and that targets were slim to nil in low-sec. The only solution I could think of was for him to join up in Faction Warfare. He hesitated saying, "i have a lot of ships an stuff and its gonna be hard to move it all across the map." So I sweetened to idea by offering to moving all his ships and stuff using my carrier.
Fast forward 2 weeks.
My friend has run into some issues and will be cutting down in his EvE time. He asks me if I can move all his stuff to a new location. I think, 'fine, no biggie.' I get his stuff via contract, stuff it into my carrier, fire up my cyno alt, and plug in the destination.
MISTAKE #1: To get to the destination would require me to make 2 jumps with my carrier. Not wanting to spend a godawful amount of time waiting for my alt's cyno to end and then move her some 27 odd systems to the final destination I ask my friend to do it.
Friend: "kay you need me to use a covert cyno or a normal cyno?" This alone should have sent up enough red flags to make me consider another course of action. Sadly, I'm lazy and in a rush. Me: "No, use a normal cyno on a cheap frigate." (there is emphasis on that last part for a reason, keep reading) Friend: "okay i know there is a special trick to lighting one, run me through this" Again, another red flag. But my laziness will not be deterred.
MISTAKE #2 The second Incursion patch had come out just days before. Reading the patch notes I interpreted the "new" cyno mechanics around POSs as applying to ALL structures, including stations. In other words, I'm adding an additional 10km to the 'drop zone' for the cyno.
MISTAKE #3 I fleet up my friend, tell him to undock, and to slowboat away from the station until he has about 15km clearance.
What I SHOULD HAVE SAID was to slowboat away from the STRUCTURE of the station until he has about 15km clearance FROM THE STRUCTURE.
MISTAKE #4 I get news from my friend (and his friend who is with him) that there is a neutral Hurricane acting aggressively next to them. I tell them let the Hurricane go. They tell me that the neutral is just docking and redocking. Ten minutes pass. Same thing. I'm beginning to get a bit ****ed off because I'm telling my friend to ignore the Hurricane and I KNOW that he isn't (for whatever reason). At this point I should have called off the whole op. I didn't.
MISTAKE #5 I get word that the Hurricane has docked again. Alright, good. Let's get this over with. Pop the damn cyno. Cyno up, I jump in.
THE RESULT
So... to run down the list... - I am a lazy, lazy, lazy person who chose a risky option against my better judgment - my friend has never lit a cyno before and my directions on how to do it were not specific enough (not to mention not entirely correct) - my friend and his friend(s) have just spent the last 10 or so minutes trash talking a pirate in local - no one has scouted any of the surrounding systems (even though I had asked them to look around and keep an eye out for hostile gangs) - a 1.3 billion ISK carrier with over 750mil worth of ships and equipment has just jumped into all this
I appear in system and immediately try to get my bearings. My first reaction is... 'where's the station?' I zoom out to maximum distance. Oh look, there's the station... and I'm 10 GODDAMN KILOMETERS FROM DOCKING RANGE (it's one of those large Caldari stations). Apparently my friend had lit the cyno 15km away from the docking range of the station. In retrospect, I was lucky I didn't land farther away.

Local begins to spike. A stream of profanity can now be heard on comms coming from me. My friend then speaks up. "What about my Falcon?" he whimpers.
I look back and lo and behold... next to the cyno ball my friend is sitting there... in a Falcon (now remember, I had told him earlier to use a cheap frigate). Me: "Are you shield or armor tanked???" Friend: "uhhhhhh... I fitted with ECM to take some of the DPS off of me!"
At this point I'm seriously considering popping him myself. But this is a side issue. Right now I am furiously clicking the 'Approach' button in a vain attempt to get my carrier to slowboat faster. I should also mention that I'm reading the distance amounts out loud to myself. ... 8340... 8210... 8090...
The pirate gang warps in. Two Hurricanes, Harbinger, Stilleto, and three or four assorted cruisers. I'm still 4000 meters away from station and my mouse is developing cracks from the stress.
I breathe a sigh of relief when the pirate gang decides to focus not on me, but my friend.
... 3000... 2800...
At this point, I decide to be a good person and lock up my friend. I put all 3 of my capital armor reps on him along with a swarm of heavy shield bots.
...2501... 2273... 2000...
My face is so close to the screen... it's unreal.
... 1283... 1125... 1031... 750...
DOCKING RANGE!!!! With no immediate threat of 'bumpage,' I focus on my friend, overloading my RR mods accordingly.
In the end it wasn't enough. With no discernible tank on his ship, my friend died a very firey, expensive, and needless death. I quickly docked up.
For the next few minutes comms are dead silent. Everyone knows they screwed up. Everyone knows that I'm ****ed. And my friend is now afraid that he won't be getting back any his stuff. Contract up. Friend: "you want one isk for all my stuff?" Me: "Yes. It's a transportation fee." Friend: "one isk?" Me: "Should I raise it to two?????" Friend: "isk sent."
At this point we all begin to laugh and give each other our apologies. After that, I logged for the night and proceeded to go out barhopping with some friends. _______________________

I lost my very first sleipnir to a station without undocking...
I'd fitted it (after saving up for ages) with a Kaikka's XL booster, Cal Navy boost amp and the rest a full T2 fit. It was 1.30AM, I was tired and accidentally selected 'reprocess' from the station menu then hit enter in irritation at the stupid box in my way.

One friday night got in a small (50ish a side) ruck in Fountain against an IT fleet. We jumped the gate, and the IT boys must have been primarying by alphabet. I lasted 5 seconds before my Battlecruiser melted, pod swiftly followed
But that wasn't the loss...
I was going away that weekend, so upon rematerialing in the clone bay, I clonejumped into my full +5 implants clone, then ragelogged at my swift death.
Back from the weekend, I log on, and theres a fleet up, and I quickly jump into another battlecruiser and off we go. No idea if it was the same *bleep*ing IT FC, but again they are primarying by alphabet, and I get about two shots off before my ships toast, and again, pod quickly follows. My annoyance at my swift death is very swiftly trumped by the sudden realisation, not only was I in my +5 clone, but in my annoyance the previous day, I hadn't updated my clone after my first death...
Total loss: 2 x 50m Battlecruisers, full set of +5 Implants, Advance Weapon Upgades 5

Lost a Curse to our one of our cableco's boxes croaking up the street, cutting off internet to our neighborhood. This happened ten seconds before a bunch of dudes jumped into us and our bubble had just been deployed, so I didn't e-warp on d/c. I'm still super duper mad about that.

It wasn't a huge loss, but... fleet fit Abaddon. It goes under "2. Knew the risk but did it anyway."
This was a few months ago when those big capital/supercapital fights were going on in O2O (Geminate). I'm USTZ so I missed most of the good stuff, but one of the fights ended up running over for a while and I thought I had a good chance of making it home from work in time and getting into fleet.
Got home in time. Got into fleet. Best speed to O2O. Gatecamp three systems out.

This was one of my friends who no longer plays but he was a bit part player anyway. Never had any iskies and sold shuttles @ a few k profit a time thinking he was making decent money.
Anyways he was in a rush so decided to autopilot home in his faction fitted tempest. Little did he realise he had forgotten to change his autopilot settings and melted in lowsec afk. He got so much stick in the corp for that but that was him through and through! So bit of 1 bit of 2 and alot of 6!!!!

I'd just splashed a lot of my decryptor money on a Sisters Core Probe Launcher, then I scanned down a lowsec RADAR. Sisters Launchers give you +10% scan strength and a hidden immunity to all autocannon fire, don't they?

was only a few months into playing EVE.
Got into an live event fighting off sansha. figured i would take my brand new super noob fit Hyperion with Meta 4 everything and faction ammo thinking im gona kick some ass. we killed off the sansha the rest of the fleet whent off to fight the Mothership and i stayed behind to loot the wrecks... took some stuff that didnt belong to me just as they came back. they destroyed the ship (rightfully so). made it back to my home station got into my noctis thinking i should go back and get some salvage. and undocked befor the GCC timer ran out i remember as i hit undock thinking i wonder what that timer is for??????

Dualboxing Proteus and Fleet Typhoon, roaming WHs for carebearing pimpmobiles.
1. Jump Proteus into WH. 2. Stealthily probe out Legion at safespot. 3. Tackle Legion. Legion tackles back. 4. Jump Fleet Typhoon into WH. Realise I've jumped it into the wrong WH. Jump back out and warp to right WH and jump in. 5. Fleet Typhoon lands on Legion just as Legion's mates also arrive. 6. ??? 7. Warp pods out, 2.6 bill down. 
I don't think it was a deliberate trap - if it was they'd have brought their gang in sooner, or had recons/T3s cloaked around the Legion - just a case of general competence on their part and sloppy scouting on mine.

I was trashed and waiting out a jump clone timer, so I decided to pass some time doing a level 4 mission. I got the brilliant idea to use a PVP fit HAM Cerberus (with 2 x sebo and a warp disruptor consuming three of the five mids). Anyway, within a couple of minutes I was swarmed, pointed and popped.
I wasn't even considering that my ship could actually be destroyed by NPCs

I was roaming 0.0 with two corp mates looking for some pew-pew. Our fleet consisted of two Vagabonds and a cynabal. We had some luck catching ratting BSs that night. So i jump into next system, and what do i see? A nice new Bhaalghorn sitting 30k off gate, and nobody else in local. We jump in engage, and wtf im neuted and both corp mates are webbed? needless to say some lucky bhaalgorn pilot solo pwned 2 vagas and a cynabal.
lesson learned: keep nano ships FAR away from Battleships with bonuses to nuets and webs.

Not too long ago..
Just got my first Tengu. I decided to run some safe lvl 4s in Empire to get a feel for my shiny new 500M ship. It worked really well - Those rats just melted, so I got overconfident, not warping out until my shields were almost completely gone on a regular basis. In one mission, my shields are going, as usual, so I align out, but fail to warp. A single rat frig is scramming me. Despite overheating the launchers, I fail to kill the frig and my tengu eventually pops. It didn't help that I forgot to bring cap charges for my booster.
I haven't gotten a new one yet. Those tengus are jinxed, I tell you!

Lost 3 ishtars doing pve in the last few months.
Category: Laziness Lost: 170 mil ISK
did a guristas military complex in an armor setup with no plate. I easily tanked the first thermal smartbomb. I skimmed over the rest of evelopedia's description, noting there was another smartbomb in the next room. I didn't notice that it was EM this time, and i aggro'd it with no shields but full armor. With roughly 2k armor and 2k structure, the 6k em smartbomb barely had enough hp to pop me. If i had waited 15 more seconds, the bit of shield recharge would have left me with a couple points of structure left.
Category: Laziness/barely awake Lost: 600m
finished up a plex with some deadspace armor hardeners and faction stuff fit all around, as well as loot from the plex. It was 5am and intel was quiet. I warped to a gate before checking local and landed on a small gang as they were jumping out. A rapier forced me through, and an afterburner + faction cloak was not enough to avoid getting decloaked.
Category: barely awake, watching tv Lost: 200m
tanking an easy plex with a setup that was not cap stable. I had turned off my repper for a few cycles and completely forgot about it until i saw myself hitting structure. A few days later this happened again but my repper cycled with 4% structure.

Back when I was beautiful, I recently got my first shiny new Legion before I had T2 lasers trained. To remedy this, I put Dark Blood Heavy Pulse lasers on because I was stupid and rich.
I then find out that lasers have poor tracking when fighting elite frigate rats. -------------------------------------------------- I am the Love Angel, Wedding Peach, and I am very displeased by your lack of love! |

I have a few good ones, they don't compare to a lot of these, but they were good learning points for me. Hell, if you haven't lost a ship, you haven't learned anything.
#1 - Lazyness My most expensive actual loss - Mission fit, poor skillz Abaddon. I'm into my first L4's and can solo them. Clearly THIS is the time to buy a battleship, but not any battleship, the shiniest, biggest most kick-ass T1 BS of all. Ooh, I remember buying it fully rigged and everything for about 75mil...oh those were the days.
Anyway...doing Blockade, which oddly enough has cost me a lot of mission ships over my career, from L2, L3 and L4. Abaddon goes boom, I'd been in it for about a month. It had more than paid for itself with casual play. The I'm-****ed-and-have-to-finish-this-misson-Abaddon I bought and fitted an hour later...That one cost me dearly.
I took a week off from missioning and reassessed my skills and later determinied I was NOT professional enough to fly that ship. Skilled up some support skillz, and have been flying an Armageddon for nearly 6 months, and it's served me well.
#2 Knew the risks New-ish to the game, flying back from jita with a full haul of various goodies in a GSC. Decided to take the short route home because...it was late. Did you know people gate camp in Rancer?! Well, now I know, and I run it in a B-runner and not a Sigil. I did get the last laugh by slowboating away and anchoring the GSC with a password while I was taking fire.

4. Barely awake. WH life. Scanned new static, scanned the farm system, which was full of sigs - more then 60 sigs, combat sites not counted. All this after 8hrs of work, i was already tired. Scanned for two hours, got really dizzy. Warp back to my POS - What the ****, WHY IS MY SHIELD ON 52%? Oh wait there are 3 BSes and assorted cruisers shooting at the POS. Oh well. I am stronted, and i cant do nothing, have fun guys. Idiots like me doubleklick when they are tired. My little Covops flies out of the shield as i logoff. POOF covops + pod.
5. Drunk (Beer mostly. Sometimes white rum. Sometimes JSger) Countless drakes + caracals. "Heeeey guuuuys, here is a drake, just jumped i tackle him...hey guys, DO NOT WARP TO ME DO NOT WARP TO ME fleet just jumped in GUYS GUYS AARGHHH *static* "I thought you said JUMP ON ARRIVAL" "What? Gatefire? I can tank it! 6. Wife/GF aggro None, she played Eve for a while. 7. General fail. Corp declares war on a "carebear corp". Two days later, the guys show up with a gang of battlecruisers + some cruisers. I die a fiery painful death because i tried to shoot at the myrm. FAIL. 2nd account undocks, warps to the gate, get bumped, scoops the loot from the first wreck, scrammed...fck, what now? "I WILL DIE ANYWAYS SO LETS DO IT WITH HONOR! *target the cruisers and shoot 2 of them*"

Flew a fully-fitted Phoenix into a Starbase siege.
2.5B ISK (at the time) gone.
After that, next biggest loss will be a recent Tengu in w-space; was in a gang doing a class 4 and got lazy and neglected to continue scanning for probes. I did get my loot out though.
After that, there's a particular dragon I had sex with. That was a very valuable loss, I couldn't even put a price on it. :(

I brought a nano ishtar to a major op back in 2008. Single dumbest thing I ever did, got desynced and died horribly once an enemy vaga charged over 150km to get to me, I was never scrammed and could not warp out. _________ EVE is like the "Fisherman's Friend" of MMOs. 2 faction fit tengu's over the course of a week. Both due to GF agro. aprox 3bil in total and almost 500k of skill points.
1st, warping into a lvl 4 worlds collide 2nd room. Forgot to activate my tank and got dragged away for something unimportant by the gf.
2nd, warped in to a bloods mission. Gf has a meltdown because she took something i said out of context and we have a blazing row. While this occurs the bloods sneak up on me and neut/nos and scram me.

I don't know how the fight exactly went, but that Adrestia had 3 Machs, a Claymore, 2 Vagas, a Cane an Curse and THREE Scimitars on his side. But it took only 3 BC and a HAC to kill his ship through all this backup.
An Adrestia, a guaranteed primary, has no business flying a gankfit with such a weak shieldbuffer. It turns a tactical asset for a gang into a liability and a logistic nightmare.

Logged back on forgetting about boosters, alt tabbed out... I then lost... no wait.. I didnt lsoe, I had half of nourv waiting like vultures tho... I then killed the customs with fofs... and flew the 70km I had been bumped off the gate!....

I ran a Gurista combat signature and its escalations, all the way out into lowsec. All went well, I popped the Dread, and then a whole horde of Gallente NPCs appeared and scrammed the hell out of me. Bye bye Zealot :(
At least the dread wreck didn't have anything of value so I didn't feel compelled to attempt to recover anything. Just got my pod-clad self back into highsec and set course for Jita to buy another ship.

When i engaged i knew the chance of a loss was huge but still well worth the effort.Did the same a few days before and could kill 2 bc's with me soloing versus 5 others and had just enough time to redock that day.
I don't mind heavy losses as long as i meant to go into pvp.I rather loose a 5B ship when i engage in pvp then loose my ratting drake while ratting

Syndication Storyline in Gallente Space, failed on the 3rd or 4th mission, where you get completely swarmed with battleships. Pretty serious DPS, should have triggered with a shuttle and came back at a distance. Course, I didn't do this, and I got what I deserve. Barely raged, it was just a "shucks, there goes a week's progress" reaction. Sucks though, we live and we learn.
Was moving to a Vindicator the next day anyway.

Category 2 I guess: should have known the risk but did it anyway. One of the first things I learned in EVE was don't fully trust anyone.
Like Dirk Magnum on page 1; our corp logi pilot was supposed to be moving all our gear from 0.0 to lowsec via his carrier. Guess he had a major fallout with the CEO and decided to take the moral low road taking his anger out on fellow corpmates by stealing everyone's stuff. He took over a billion in ships and mods from me alone.

Go back to highsec because some wt logged on.
Dialog on jump, don't read, click ok.
Appear in highsec with GCC, Concord arrives, scram web, pewpew -> One vaga down
So I go to the nearest tradehub, buy a new vaga and proceed to the wartargets area. The wartarget is waiting a bit of the gate in a myrm. (yes bait, I know)
We engage and local spikes -> Second vaga down.
That was enough for the evening....

I once took a courier contract to a 0.0 outpost where I couldn't dock. There was a little niggling voice screaming "It's a Scam!" completely ignored it and lost 120 million ISK in collateral but got a small standard container from the package. The contract creator seemed to be an alt created specifically for scams.
Within hours of being able to fly a Maelstrom at all, I took one through a wormhole against a "particularly hard" sleeper site. Managed to last surprisingly long but had no way of escaping the sleeper neuts and warp scramblers. It was interesting trying to recover my stuff with a Zepher. Of course I didn't remember insurance. It was about a year before I went through a wormhole again. 100+ mill
Latter, I lost another Maelstrom underestimating the COSMOS deadspace complex. Turned out the insurance had run out the previous day but the "You're insurance ran out!" notices don't always trigger when they're supposed to. 100+ mill
Was moving planet goods out of Low-Sec in a mammoth, a guy followed me through a stargate. I watched him so intently I didn't notice when my gate cloak wore off. I had a improved cloaking device, but never got to use it. I also double clicked my cloak on my Prowler once, turning it off again accidentally, received damage but didn't actually lose the ship.

And my loss. Something about 1.5-2.5bils i guess. It was Nightmare with faction fit. Reason: lazyness. I went with my main to shoping and decided to run Drone Patrol with my second alt (who has bad drone and tanking skills and never tried soloing).... He got scrambled by 3-5 frigates and noone in corp was fast enough to resque him. My main was 10+ jumps away by that time.....

Later i have lost tengu (wrong move in final room of 10/10 escalation, get neuted, scrambled and melted). Good luck though: i took all expensive stuff from wreck. And rattlesnake: while shooting drone mother i went to have dinner with my wife. For too long time.....

1) 2.4 billion ISK Tengu to an instalocking high-sec WT HIC+Rapier+Bhaghlaorn
2) 800 million ISK CNR after clicking "warp to and dock" on a station as the GF was yelling at me to get ready for the beach. I alt-tabbed and the ship never docked. Apparently, it sat outside a 0.0 station for 2 hours before a Tri gang finally killed it. She was a ***** about it too.
3) 600 million (2x Slepnir in about three days jumping blind into EC- and getting scrammed/webbed somehow before I could burn back to the gate)
4) Sabre (only 60 mil or so) to a Vaga when I was ****faced. I started about 200 km from it too.
5) 150 mil Raven when BS were sorta a big deal in HED-GP after slowboating safely without a scout for 25 jumps in 0.0.

- Was in State Protectorate to avoid paying taxes (too lazy to create my own corp) - Girlfriend aggro in the middle of a mission, decide to warp to the sun - Come back 10 min later, find myself attacked by an Abaddon and another ship, already in low armor - Try to escape, ship does not respond because my bandwith is saturated by the downloads I started earlier - Tengu explodes, try to save my pod, big lag, yeah I'm dead..
Estimated total loss: <1.5B

Did not pay attention for 1 minute due to wife and child messing around my desk ( my little girl sometimes find it fun to push the power off button of the PC while Im on it 
When I come back to my screen, Vargur shield is almost down and Im scrambled by 2 elite frigs, killed the first one then I explosed when the second one was in hull.
Needless to say I was not too happy about it, my wife come back in the room 5 minutes after and see me a bit upset, she ask : what's wrong ?
I say : nothing , just lost something , was not going to say I was ****ed off because I lost a valuable virtual spaceship 
Cancelled the mission than warped back to get the can and salvage the wreck, just crap in it, lost as well the 3 RF gyros -))

I bought a Cynabal and was trying it out. So, I was running around lowsec when I found a shuttle on a gate. Thinking that it was a .4, I blew up the shuttle, and then died to CONCORD.
200 mil Cynabal for 10k ISK shuttle.

My worst one to date. Moving all my **** out of low sec once our corp had closed. Had to jump through one low sec system, that usually had the high sec gate camped. Was talking on Skype to a friend, while checking the area with my alt. Nothing in either system. So I send my full to the gunnels Orca to warp to gate. Orca's take a LONG time to warp. As I arrive on gate, someone has entered the system. Because I hadn't been paying attention (talking on Skype) I assume he had jumped through the gate. He pointed me right away. I didn't want to jump into his buddies. So I put ECM on him and tried to warp back to station / safe. But I was wedged on the damn gate and couldn't move. Local spiked, and the rest of his buddies warped in and BOOM.
My pal on Skype doesn't play eve, and was perplexed by my panic.
I came back in the only ranged fit ship I had (Rook) to try and pop some cans they were sifting through. Only got one, who knew Abaddons can hit that hard at 60km.

And one of the worst losses I have been privy too. A couple of my corp mates had just brought some pimped out ships. One definitely was a faction fitted Sleipner and the other maybe similar. Crystal sets in clones and everything, sparkly new from Jita. We jump into Kourmonen, WT cane jumps in from Auga, they start to light him up. He aggresses (not a good sign). And 30 Man WT fleet jumps in. Super lag fest, all three of us podded. I was just in a T2 Frigate, but clone wasn't up to date. That put the damper on PVP for a bit.

Twas a mix of 2. Knew the risks, 3. AFK and x. I was dumb and hang my head in shame.
I flew to a lowsec system, where I wanted to scan planets for ressource compositions. Knowingly, that I do not want my fitted, rigged thrasher (yes, I AM a new player) destroyed, I just jumped into a shuttle and scanned away.
In the middle of scanning I remembered that I do have remote scanning trained a bit further, so I scanned numerous planets while sitting next to a planet in the system at the point where I jumped in. After about 5-10 minutes a frigate jumped in and tackled me. The rest is obvious. No conversation.
I lost about 1500 LP and 1.5m ISK in cheap beginner implants and a 40.000 isk Shuttle, uninsured. I continued scanning from my home base afterwards.
I foresee worse (well, actually bad) ones in the future.

edit: I didn't realize I'd already posted in this thread. Anyway a repeat of the theft mentioned earlier is below, along with a bunch of other figures from other losses.
Categories 1 and 2: Lost a billion to theft because I trusted the wrong guy. Several people in the corp lost similar sums during the same event. The corp was relocating and this guy had a freighter and well you can guess the rest. I used it as an opportunity to get back to basics and stop filling my hangar with shiny things I never undock with anyway.
Category 1, 2, and 3: Autopiloted Dramiel through a 1.0 system with a -3 personal sec status. Fortunately it was unfitted so the loss at the time was maybe 75 mil.
Category 3 and 4 and sometimes 5: Dominix, Armageddon, Apocalypse, Maelstrom, another Dominix, and a Raven all lost in missions because I CBA to notice a new spawn of enemies that show up. Total loss probably under a billion. I've only ever lost one battleship in a mission that wasn't because of this reason.
Category 5: Bought a Vargur because I thought I was on the test server (I had both servers open at the time.) It may have registered with me that I was paying hundreds of millions of ISK for it, but I probably thought to myself "I'll just suicide a 100 ISK dreadnaught later for insurance." Even though I can't fly Marauders on TQ I was able to recoup about 95% of what I spent on it after putting it back up on the market. I make rash decisions on whisky so I'm guessing this was Canadian Club 6 Year Old.
Category 3 and 6: Lost a Worm that I had just bought to belt rats. Wasn't girlfriend aggro per se in a romantic sense although it was a close female friend who I got preoccupied instant messaging. This was pre-buff Worm though so the loss wasn't even 30 mil.

Not sure if I was drunk or just stupid.
Not a huge lost but one that sticks with me more then others.
I had just trained up for a Falcon. I was still getting a feel for the ship. I wanted to kill something but none of the corpies were on. So I jumped into Gal low sec space and started searching. I practiced hunting down targets, locating on D-scan, warping into belts with out getting decloaked. All that fun stuff.
After a few jump around low sec I found a nice juicy target. There was a covetor mining in a belt all by his lonesome with no one else in local. I get my ship into range, decloak, scram, jam, and open fire. My MASSIVE dps worked quickly through his shields, then his armor. About halfway into his structure I was salivating in anticipation of that satisfying sound of ships going pop when suddenly my computer lags. I regain framerate to find myself in a pod. WTF.
Then I looked up to see the 0.5 next to the Security Level.
Thankfully the guy was too scare to scoop my loot and I was able to recover most of it using an alt.

Recently joined the corporation "Degenerate Corp", in the allience "Get Off My Lawn" out in 0.0 space, I trusted and have known them for nearly 8 months, set up a P.O.S. for them. I had 4 ravens, 1 noctis, and a buckload of salvage in the POS hangars.
Was out mining ABC ores in allience held space in 0.0, after a few days I decided 0.0 wasnt for me concerning mining operations, 7 days later the corporation decides to tell me they have a tax rate on mining. (Yes, because you can totally anylise how much said corp member is mining, accurately). Except heres the catch, its not a publicised tax rate, its stated on a cargo container in a corp hangar in 1 station, one that nobody uses.. smelling fishy right?
So anyways I asked them to give me 7 days for their NEW tax rate, as im now being taxed on ORE I mined well in excess of 7 days ago, instead they decided to kick me out of corp.
I lost a few ravens.. and a p.o.s, but so what? I gained something that day, I could have invested more time in these guys, I really did trust them & I didnt lose my honor, I explained the flaws in their system.
A few days later, an insider opens comms with me, it turns out the Director wanted to sell my ravens and loot for a PLEX card, due to the corp being low on capital due to POS fueling.
Bottom line is, losing 500mil of assets turned out to be a profit. Signature is too large. Maximum permitted signature size is 400x120 px. Spitfire
