For over ten years I do a scan once every year from a bootable medium. Then I reinstall. It's a habit.
In all these years I got something ONCE, straight from microsoft.com, which I find hilarious. I was able to tell, because my system started to behave differently. Now with the SSD it's a tad more complicated, but with HDDs only it was really easy for me to notice differences in response times ... as example.
I went through the taskmanager to check if there's something that doesn't belong there and there was. Killing it didn't work. It reloaded. I digged a bit and found the program on the filesystem. took me a while.
deleting it wasn't possible... so I wrote a batchfile that kept trying, deleted the program from memory and the batchfile deleted it accordingly on the HDD.
Then I made sure it's really gone.
That was once in over a decade. I'm sure I'm safe. My weird mind lets me notice even slight differences in respond times or when cpu or ram aren't at values they actually should be.
Imagine that.... if you don't download ****, don't go to ****** websites and don't install crap, while actually knowing the machine and it's behaviour and timing... then you don't need AVs at all.
Besides that, my system is always up to date and Microsoft security essentials seem to be working. I can't tell... they never tell me that something's wrong.
I understand that I'm a rare case, though.

My brain is weird. I consciously notice even slight changes in my surroundings or people. I always get hilarious reactions when I spot weight differences of a single kg on women I see every day and they go check and confirm.
It's quite demanding under certain circumstances and no fun at all.

I'd suggest dropping saying "you", when it's not actually fitting.
It's not "you can usually tell", it's "one can usually tell", for example. Many more of these, sadly.

also means i have to have a completely dark room and as little noise as possible to sleep....
mostly noise, now i think about it. i once heard a whine while in my dads car, whenever he pulled left. no one else heard it until it got worse and needed looking at. stuff like that.

i only need silence when i'm trying to sleep.
i have to have music on when i'm working, and when i'm playing games where sound isn't as important, like EVE. if i'm playing something like Fallout new vegas, for example, i like to hear when something is behind me, or the click of a trap, or something like that, so i kill my own music adn have the ingame radio at a reasonable level so i can still hear those things.
with people, i notice some differences, though not as acutely as weight gain/loss. i notice the usual, things like changes in hair cut.
i'm mostly sound, with some visible differences, like if something is moved slightly out of place, or slightly crooked.
my main one, and most annoying, is in social settings, i will notice people, and what they're doing. coming, going, what they're carrying, who they're with, if anyone. couldn't tell you what they look like though, but i could probably tell you where they sat or something.
i know someone else who does that too.
