I was in my Deimosā
I warp to a gate in 0.0 and suddenly see a 15+ reds coming out of the stargate. I double tail back and warp to a safe spot in order to monitor the activity. I hear other locals saying that the reds have deployed a warp bubble and are now camping the gate. I decide to move to my sniper spot near the gate but select the wrong bookmark and warp 25KM to the gate right inside the warp bubbleā.
I must have pressed CTRL+SPACE like 80 times before getting back thereā.
Guess what happened next.

Epic Fail 1: Getting myself killed in a (expensively) kitted Stealth Bomber at a bubbled gate because I forgot about the... dun-dun-dun!!! cloaking device. In my (weak) defense the lone camper had a nutters locktime.
Epic Fail 2-X: Loosing expensive missioning ships due to getting distracted/going semi-afk and then getting popped by three respawning frigates, or whatever else happens. I think I lost about six ships like this by now.

In my first corp about a year ago, I was going to help support a few cyno jumps. I got the name if the system and typed the first 3 letters of the system into search and set destination; 25 jumps.
I am like "ok it will take my 20 minutes to get there." I get there and I ask, which station, and they say last one on the list. I select and warp. No one is there. I then check local and no corp mates are in local. I ask again for the system and corp mate links it. I had the wrong system. The new location was 22 jumps back in the opposite direction. 47 jumps for a cyno op.
Well the guys applauded my dedication to the corp
Other fail is turning of the warning pop ups to early as a rookie pilot. That caused a number of losses and much rookie aggrevation
EDIT: No supper embarrasing ship losses yet. They have all been expected.

I was once roaming low sec, noticed a mastodon in belt started Pew-Pew only to get killed within seconds... Wondering what happened I looked at logs and noticed faction police killing me... moved my eyes to the left-upper corner of screen and noticed 0.5
Lost nice ishtar.

This one time (actually several times) I was fighting fleets outnumbered with this char, and my older alt in t2 fitted battleships for dps/buffer/RR and I got permajammed. That was pretty epic fail for me.... until the jams stopped and their jammer died Then all hell broke loose and it was epic fail for them

That reminds me of a very WTF moment I had about a year ago. I was missioning, just minding my own business, then shot ONE rat. Concordoken. For no apparent reason. At all. Expensive rigged Abaddon gone.
My face at the time was likely befitting of an epic fail at least.
Thankfully I got it reimbursed. Still using the very same Abaddon for missioning now.

Tried to solo a Drake with t2 drones in a Wolf. Sometimes i'm like a dog locked in a butchers overnight.
What makes it pure fail is the fact he was passive tanked and i was outstripping the regen. I don't want to set the world on fire |

Rancer for me too,
I took my first, shiny, new Iteron III that I was extremely proud of, fitted about 10M of stuff in and on it and set my autopilot to take me somewhere between the Federation and past Rancer. As soon as I decloaked, an Onyx locked me down and a Rokh two-shotted me.
I haven't set foot in Rancer since

Epic fail on my part
More recently, though: While drinking, I decided it'd be a great idea to jump my alt through low-sec. I figured: "Hey, he's in a Cheetah, nothing can catch him!"
Well, a few jumps in, I stopped checking local and forgot to use my cloak, didn't scan or anything... Warped right into a gate camp Now that is fail.

I was very Very new to EVE I was getting ready to JUmp in to Low sec and a warning popped up about low sec. I was like yeah sure what ever.. I Jumped into Aammake..4 Sec later clone station.. A wonderful way to loose my 1st cyclone that i couldn't even afford to insure... Fail..Fail!

While I haven't had any really epic fails worthy of this thread I have been close a few times. Like filling up my hauler in Torrinos, setting my path towards Nonni, warping to the gate, jumping thru only to discover that I warped to the wrong gate and are now in EC-P8R in a cargo expanded hauler with a few 100m in the cargo.
Luckly it was one of the few times the gate wasn't camped and I managed to get back into Torrions alive.
A corpmate had an epic fail. He got a Barrage S BPO from a research agent, put it in the cargo of his shuttle and turned on the autopilot down to minmatar space. In my early noob days, I went oursulaert and bought my whole wallet in named mods (around 10m). Later that night I got drunk, ran a couple missions and back to station reprocessed the whole hangar...

One time, while I was in celes, I was leading a 0.0 gang in vale.
So we fly around for a bit. We find nothing, we were based out of empire at the time, so we get bored and start heading back towards obe.
I bump into darth along the way and he's like "so is it clear? Because I want to test my setup on you."
Of course, me being a jackass, I told him it was, even though I hadn't scouted behind us properly.
So me and darth are sat on a gate in the middle of vale, blasting each other like idiots, when a BS gang lands on us. We managed to jump through to the other side, at which point they follow us and proceed to gank us. I solo killed one of their geddons while they were shooting darth though lol
Moral of the story is, don't have a fight with a corp mate on a gate, in the middle of 0.0 with enemy gangs flying around.
Then there was the time that me and b00gieman went on a 0.0 op with just the two of us in BS, and b00gie got killed by NPCs (WTF?!) after flying around for ages not finding a soul.

Once upon a time, i finally had enough cash for a rhea. Actually, i did have the cash, but didn't want to pay too much, so i waited until they dropped to 4.5b.
But this isn't exactly the story of my rhea, it's the story of two general freight containers with loot.
Anyways, i was there, with my freshly bough rhea. The plan was like this:
Fly to a highsec system near 0.0, where the two general freight containers were. Bring stuff from the highsec system which was needed in 0.0, then jump to 0.0, exchange the freight, bring the two containers back to highsec via a lowsec jump, haul them 20 jumps in empire, return to highsec base.
Pretty complicated, eh?
Well, it would have been a lot less complicated, if i actually named the containers, and didn't forget them in empire when i jumped to 0.0. Of course, i accidently jumped to the wrong cyno in 0.0, didn't have enough fuel to get to my destination, and had to jump to another station, leave the ship, haul fuel, and jump to the final destination, only to discover that i didn't take the containers with me.
40m in fuel.
So i went to highsec again, and of course...forgot to take the stuff i wanted to take to empire with me.
+10m in fuel, so 50m for a trip of one hour, without any cargo. When i jumped out to lowsec, on a pos, a ceptor came out of warp just as i entered the forcefield to recharge cap and local spiked. Yikes..
So, when i was finally done with hauling, i had burned about 100m isk in fuel, wasted 5 hours of my life, nearly got killed by a bs fleet, and jumped to an unscouted cynogen in a backwater 0.0 system.. ----------------------------------------------

I don't think so. For some reason he thought it would be a good idea to take on a high value NPC with his pvp setup. I think they were tougher at the time. Such things are not entirely unusual for b00gie tbh lol

I once set an AP to Jita, all green....only to hear pew-pew a while later and see my shuttle and pod killed in Rancer. I am pretty sure AP changed routes on me mid-flight (I had seen it in the past on occasion).
Luckily it was implant less clone.

1. Aggro somemone at gate 2. Getting into pod 3. Hitting warp repeatably to safe pod 4. Getting podkilled cause it didnt work 5. Going to bed 6. Waking up next day , sober not drunk any more. 7. Looking at fraps and seeing that i was not hitting the warp but jump button. 8. Realizing I just lost a HG slave set and +5 hardwires. 9. Thinking jeez you nublar.

Then One jump from 0.5 sec (was in 0.4) I figure, heck I'll go to an asteroid belt and mine a few rocks for the extra Isk.
Now bear in mind I just read Halada's Mining gudie and read about how people used to mine in BS's and the like before Barges and exhumers came out, so I had a one track mind...
I get to the belt and see a single lone guy. I type Hi in local only to realise it was in corp chat... Right click Showe Info to see he is in a HAC... WOW, look at those stats. he must be able to get a LOT of T2 Miners on that thing, I think. Wow, look at all his mining drones....
You can guess the rest...

Hmm, only last year I think, I was in a PoS bash fleet. There we were, all aligned ready to warp to a planet or something, perfectly shooting at the thing when suddenly the enemy fleet was warping to the grid. The FC (Gob) ordered us to warp off.
I tried to warp but for some reason I had a station selected... and was realigning for ages. Pew Pew boom. Mine was the only ship lost that night.

I am 100% positive that right there has happen to more than just you. I believe that 50% of all ship loss in EVE is the direct result of playing wasted!
This thread turn out way better than i could have ever hoped.. There some really good ones here. Thanks guys/gals.
